The Urgent Community Response Team based in Lincolnshire have an exciting opportunity for a Therapy Assistant Practitioner to join our team. We are supportive and fun team who are passionate about supporting patients to remain in their own home where able and prevent unnecessary admission to hospital.
UCR covers 7 day working (8am - 8pm - be available forearly, late or long shifts and to work flexibly)
Support individuals to remain in their own homes utilising aperson-centred approach whilst taking account of theenablers, carers and relatives' opinions.
The service covers Lincolnshire with various bases throughout Lincolnshire. The location of the staff base can be discussed at interview.
