We are delighted to be able to offer an exciting opportunity to come and work in ourUHDStroke&NeuroEarly Supported Discharge (ESD) Team as a Band 3 Admin Coordinator.This service worksclosely alongside patients with stroke and neurological conditions,in their home environment,toprovideintensive rehabilitation and promote independence after discharge from hospital. This permanentroleiscritical insupportingthe multidisciplinary ESDteam,to deliver effective and coordinated care for individuals with stroke and neurological symptoms.

We are looking for an enthusiastic and motivated individual to work in our team and applicants should be able todemonstratea high levelof flexibility within their work, learn quickly and multi-task.

**Base Location**: Bournemouth with cross site working with Poole

**Interview Date**: TBC

Duties will include coordinatingadministration forthe ESD serviceand to link with the in-patientreferringteamas necessary,toprovidea seamless, high quality rehabilitation serviceforourpatients.

Thisrole will alsosupport the ESD Team Leader in coordinating the general day to day administration of the team.There will also be some data inputting involved for national audit purposes.
