Thee post holder will work supportively and collaboratively with the Clinical Team Manager. The post holder will undertake the care coordination of a defined caseload. Carry out assessment, formulation, care planning and delivery of direct patient care with appropriate supervision, ensuring continuity of practice and working within a multi-disciplinary and multi-agency environment. To implement risk screening and risk management, Including the development of a crisis plan in consultation with the user, carer, and multi-disciplinary team.

Ensuring all service being delivered is co-produced. Support the development and implementation of recovery focused interventions as well as recovery focused groups, to patients with at-risk mental state and their carers in line with NICE guidelines. Initiate, create and maintain therapeutic relationships with patients and carers and relevant others. To keep all relevant statutory and non-statutory agencies, including primary health, fully involved in the care plan process, through verbal and written communications.

To support the ARMS triage team, in order to determine the need for treatment and the nature of treatment required for those being referred into service. To liaise and to negotiate with other establishments if not ARMS, and to seek out suitable care pathways. To support the duty system and advising to other health care professionals. To support service training to other professionals and agencies that may come into contact with people with at-risk stage presentations, supporting psychoeducation of the signs and symptoms of illness and how to refer to the service Support and develop the principles of joint partnership working and to be pro-acting in fostering positive inter-agency relationships and information sharing.
