**Mental Health Nurse / Social Worker / Occupational Therapist**

Hybrid working, and a warm welcome accompany this opportunity to work creatively with Young People and their families within the Community based Youth Team in Kings Lynn, West Norfolk. We are a multi-disciplinary team promoting positive Mental Health and providing treatment interventions for Young People aged 14 to 25 years. The role is for a fixed period of 12 months to cover maternity leave.

We strive to be dynamic and creative and are dedicated Mental Health Professionals, focussing on the needs of Young People providing a needs-led approach. We are seeking like-minded clinicians to work alongside us in supporting Young Peoples Wellbeing and Mental Health.

**Main duties of the job**:
***Duties will include, but not exclusive to:

- Triage of new referrals
- Conducting Assessments
- Supporting and being part of the Duty system within the Team
- Individual Caseload holding and management
- Delivering evidenced based mental health treatment interventions to young people both on a one-to-one basis and in group settings both face to face and using online mediums such as Attend Anywhere, Microsoft Teams and Zoom.
- Multi-agency working; including working alongside families, acute hospital, primary care sector, social care, education and voluntary services
- Active contribution in team meetings and reflective sessions so that the team can learn, grow, and improve the service we provide
