The Community Team Manager for the At-Risk Mental State Team; Norfolk and Waveney would like to tell you about an exciting addition to this new service which has opened an opportunity in the King’s Lynn area, and will require travel to the Norwich area.

In expanding our team, we would value your expertise. We are looking for Nursing degree or Equivalent professionals, with a relevant mental health background to support care co-ordination, to hold a caseload and to plan the care and support for these individuals.
- Willingness to support new team development, allowing for flexibility in the development stages. Be adaptable to service demands and developments as under the direction of the Community Team Manager.
- To manage a caseload and act as care coordinator for people who are experiencing at-risk mental states. To undertake the planning, assessments, formulation, risk assessment/s, care planning and delivery of direct patient care with appropriate supervision.
- To support the prevention or delayment of a first episode of psychosis, by supporting people in all aspects of their lives. This includes ensuring physical health assessments take place, wellbeing support offered, support around social functioning, employment/education, as well as supporting those who support.
- To support and carry out assessments. Willingness to complete CAARMS training and to regularly utilise the CAARMS screening process in supporting the assessment, and formulation of needs.
- To build therapeutic alliance and supporting therapeutic relationships.
- To support the development of continuous service improvement through co production and collaboratively working with individuals with lived experience and their carer/s.
- To maintain high standards of clinical work in compliance with NICE guidelines for at-risk mental state teams and the care quality commission.
- To support the specific quality standards for ARMS.
- Liaising with other health and social care and education professionals to ensure joined up care

- The post holder will undertake the care coordination of a defined caseload.
- Carry out assessment, formulation, care planning and delivery of direct patient care with appropriate supervision, ensuring continuity of practice and working within a multi-disciplinary and multi-agency environment.
- To implement risk screening and risk management, Including the development of a crisis plan in consultation with the user, carer, and multi-disciplinary team.
- Ensuring all service being delivered is co-produced.
- Support the development and implementation of recovery focused interventions as well as recovery focused groups, to patients with at-risk mental state and their carers in line with NICE guidelines.
- Initiate, create and maintain therapeutic relationships with patients and carers and relevant others.
- To keep all relevant statutory and non-statutory agencies, including primary health, fully involved in the care plan process, through verbal and written communications.
- To support the ARMS triage team, in order to determine the need for treatment and the nature of treatment required for those being referred into service. To liaise and to negotiate with other establishments if not ARMS, and to seek out suitable care pathways.
- To support the duty system and advising to other health care professionals.
- To support service training to other professionals and agencies that may come into contact with people with at-risk stage presentations, supporting psychoeducation of the signs and symptoms of illness and how to refer to the service
- Support and develop the principles of joint partnership working and to be pro-acting in fostering positive inter-agency relationships and information sharing.
