Paediatric Diabetes Specialist Dietitian

To have responsibility for providing a comprehensive dietetic service to paediatric diabetes patients treated within the Oxford University Hospitals NHS Trust (OUHT) at the Children’s Hospital, John Radcliffe Hospital, Oxford, and in Transition Clinics at OCDEM, Churchill Hospital, Oxford. This involves working as an integral member of both the Dietetic team and the multidisciplinary team to provide a flexible, prioritised service based on clinical need and in line with Best Practice Tariff.

To educate and counsel children, their parents, family and carers on the management of diabetes from diagnosis until discharge to adult services. To proactively case manage and anticipate future care needs to develop management plans accordingly.

To provide advanced clinical skills and advice demonstrating a sound understanding of the issues related to the identification, assessment, diagnosis, treatment and management of the client group.

To offer long term support to children with diabetes and their families particularly at times of treatment change, times of crisis such as illness, times of lifestyle change such as residential school trips or moving on to secondary school.

To maximise clinical outcomes for all children with diabetes by matching dietetic management with optimisation of insulin therapy and practical skills for each individual.

To contribute to the implementation of relevant research into practice contributing to the further development of evidence based research and audit.

To educate, train and disseminate information to healthcare professionals including clinicians, nurses, allied health professionals, dietetic colleagues and students about the dietetic management of paediatric diabetes.

To be an active leader of the broader dietetic team and specifically the paediatric dietetic team to meet the dietetic needs of all children, particularly those with diabetes, attending the Children’s Hospital.

Our friendly and vibrant Children’s Therapy and Gait team consist of approximately 80 staff working across 5 professional groups (physiotherapy, occupational therapy, speech and language therapy, dietitians, healthcare scientists). Together we offer a mix of inpatient and outpatient services to our patients across 4 sites (Nuffield Orthopaedic Centre,, John Radcliffe Hospital, Douglas House, Horton General Hospital. to provide world-class collaborative multi-disciplinary care. We are fortunate to work within a large teaching Trust housing many tertiary and regional clinical specialities which encourages staff development, research and quality improvement initiatives.

Children’s Therapies sits within the NOTSSCaN directorate within Oxford University Hospital’s NHS Foundation Trust. There is a strong learning ethos, and the team regularly host students across all professional groups in addition to supporting continual professional development of its staff.

**Clinical**:
Be professionally and legally accountable for own work and caseload. To support other specialist dietitians, nurses and junior doctors as required.

Work autonomously in the provision of a comprehensive nutrition and dietetic service to the paediatric diabetes service at the Oxford Children’s Hospital

Deliver dietetic provision to ensure a comprehensive specialised paediatric diabetes dietetic service is provided to approx 340 children attending the Children’s Hospital and Horton General Hospital. This could include:
Outpatient clinics

Dietetic annual review appointments

Insulin pump starts

Transition clinic

Home visits (joint or on own)

Group education

Work as an integral member of the multidisciplinary team to ensure a holistic approach to patient care. This involves reviewing patients jointly with the paediatric consultant and/or paediatric diabetes specialist nurse during out patient appointments, on the ward or in the patient’s own home. Participate fully in the problem solving aspects of the consultation which may involve:
changing insulin doses, regimens, and the types of insulin used;
changing the daily routine of insulin injections to fit with an individuals lifestyle and diet;
advising on the management of hypoglycaemia;
interpretation of biochemistry results;
discussing dietary issues associated with therapy changes.

Provide on going education for the patient and their family on the ward, in out patient clinics, at home and on the telephone as dictated by the needs of the individual and according to the agreed standards in the paediatric diabetes team guidelines on care for the family and child with diabetes.

To assess the nutritional status of individual patients based on anthropometric measurements e.g. weight, height, weight history as well as biochemical parameters, diet history and other validated methods of nutritional assessment as appropriate.
