Nursing Assistant Children’s Diabetes

The Oxfordshire Children's Diabetes & Endocrine services covers a wide geographical area, with clinics at the Children's Hospital, John Radcliffe site, and additional diabetes clinics at both the Horton Hospital and Churchill Hospital. The nursing teams are based at the John Radcliffe site and the post holder will work closely with these teams. It will be required that the post holder works in clinic in Oxford only.
To coordinate the care of children and teenagers while they attend the diabetes and endocrine clinics, including routine clinical assessments and downloading of monitoring equipment. Care of the patient through the system is to be patient-focussed and efficient. Clinics are mostly at the John Radcliffe site with 1 per month at the Churchill Hospital site.
To plan and book the diabetes appointment system for the Horton hospital children/young people.

**Key Duties and Responsibilities**:
Clinical
- Before the clinic starts, ensure that all paperwork is available
- Order blood and urine tests as required/advised
- Welcome patients to the outpatient clinic and act as a coordinator of patient care through the clinic; observe where patients are in their pathway and communicate effectively with the multi-disciplinary team, maintaining confidentiality at all times
- Provide a high standard of care to patients in a calm manner, ensuring appointment times are adhered to fairly
- Assist the nursing, research and medical teams with any other patient management during attendance at clinic
- Ensure that all procedures are carried out in comfort and safety, taking into account issues of privacy and dignity
- Act as patient advocate during the clinic appointment
- Communicate with staff in other departments in a professional manner
**1. DIABETES CLINIC**:
- Weigh, measure and take BP reading for patients according to the need for the relevant age
- Assist the patients to carry out finger-prick testing for a routine blood sample on attendance at the clinic
- Carry out a measurement of HbA1c using an Affinion/ Vantage DCA machine in a clinic room (full training will be provided)
- Make the result available to the medical and nursing staff seeing the patient in a confidential manner
- If necessary repeat a sample or carry out a duplicate laboratory sample if required
- Download the patient’s blood testing meter and insulin pump and continuous glucose monitor (if appropriate) onto the clinic record system (full training will be provided)
2. ENDOCRINE CLINIC
- Take and record anthropometric measurements with precision
**Additional Support to the Diabetes Nursing Team**:
- Manage all aspects of the diabetes clinic preparation including producing clinic sheets from Twinkle database/EPR as appropriate
- Typing up of handover sheets, organisation of blood and urine bottles.
- Monitor and maintain the clinic paperwork, including stocking up of protocols and guidelines (Children’s Hospital, Horton and Transition)
- Maintain and order equipment for diabetes clinic, including stocks of injection, pump and monitoring equipment from external companies
- Ensure that filing is carried out in a timely way in the diabetes nurse office
- Maintain clinic list for Transition clinic, including reminding patients of their appointments and setting in place a system for checking discharge planning
- Assist with the programme of diabetes training in schools in an administrative capacity together with the Diabetes Specialist Nurses for schools
- Enter information on the Twinkle diabetes database system at the request of diabetes specialist nurses
- Receive and take appropriate action on internal and external telephone calls. The post holder will be expected to handle all calls in a confident, courteous and professional way ensuring any messages are communicated promptly and accurately
- Liaise with the Diabetes and Endocrine Administrators to maintain good communication
