The salary for this permanent Clinical Transfer Co-Ordinator job is £33,000 - £36,000 per annum.

**Main Duties**:

- Drive pull through of patients by maintaining a clear understanding of available capacity so all appropriate patients identified can transition into the support and care of our multi-disciplinary team in the virtual ward environment at the earliest possibleopportunity to meet capacity and contractual targets, whilst maintaining high degrees of safety and delivering excellent levels of care (Forums include ward rounds, 1:1 stakeholder meetings, visibility on site, leverage via CTM / SCQM)
- To complete a comprehensive clinical assessment of potential patients reviewing their holistic care requirements to determine their suitability for transition to the virtual ward; this face to face review is underpinned by a detailed review of patient medialhistory, close liaison and clinical handover with the Trust MDT and is documented in real time via appropriate documentation.
- Establish and maintain exceptional stakeholder (internal and external) relationships to embed an easy to access, high quality, value adding service that becomes a long term extension of the trusts ‘business as usual’ offering which expedites patients’ returnhome and recovery.
- To gain high levels of trust and confidence with both patients and their carers through exceptional communication and high-quality discharge plans to deliver a smooth transition into the virtual ward
- To proactively share feedback and insights that could enhance or improve service levels with the management and trust team. Seek out and share feedback for continuous improvement around patient experience, service quality and efficiency.
- Liaise with referrer / consultant to gain approval (written consent / approval via the approved documentation) to commence Sciensus Service and projected care plan and discharge goals including frequency, duration, type of clinical interventions.
- Compete a comprehensive holistic face to face clinical assessment of potential patients to determine suitability for transition to the virtual ward; includes review of medical history, mental capacity, support network family / other agencies, explanationof positives around virtual ward model, risk assessment and document all findings accurately on appropriate forms and technology (initial patient assessment form on the FlowFinity system) at point of assessment.
- Based on the clinical assessment develop a personalised care plan and agree discharge goals that addresses all care needs and is within the scope of the agreed operational manual

**Requirements of the Clinical Transfer Co-Ordinator**:

- NMC registered and HCPC registered
- Experience of working as part of a MDT
- Experience of carrying out multiple face to face patient assessments
- Experience of influencing senior stakeholders / clinicians to progress new ideas / ways of working
- Previously undertaking holistic clinical assessment skills, including comprehensive risk assessments, and clinical reasoning skills -(ideally as a core aspect of job role)
- Strong interpersonal skills, establishes rapport quickly, persuasive, motivates people, driven to make things happen, positive approach to work, resilient
