The Surbiton & Chessington PCN is looking to hire a Care Coordinator. This is a fulltime role and will work predominately across 2 GP Practices (Hook Surgery and Chessington Park Surgery) both based in the Merritt Medical Centre. At times there may be a need to work across other Practices.

This role would best suit someone who has either Primary Care or Secondary Care experience. The salary will depend on experience.

The purpose of the Care Coordinator role is to play an important role within a PCN to proactively identify and work with people, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of careand support across health and care services.

Key Responsibilities

A Care Coordinator may be required to deal with patients and, if appropriate, their carer, before or after thepatient’s consultation with a clinician or other healthcare professional. This will include liaising with Care / Nursinghomes and other health and care providers.

Working closely with the patient and their clinician or other healthcare professional, the Care Coordinator co-ordinates patients’ healthcareand directs them to the appropriate service to ensure that they get the most suitable care from whatever health or social care provider is appropriate.

The Care Coordinator’s role requires them to be able to work with, and understand the roles of, a variety ofdifferent people working in the practice and across the PCN including GP’s, Nurses, Clinical Pharmacists, Healthcare Assistants, Social Prescribing link workers, Physiotherapists and Pharmacy Technicians.

There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels and the availability of a Social Prescribing link worker within the PCN:
To work closely with practice and PCN healthcare roles, the Care Coordinator is to identify and work with a cohort of people to support their personalised care requirements

To collate all of a patient’s identified care and support needs and review the options to meet these needs and bring them into a single personalised care and support plan (PCSP) in line with best practice

Participate in the project work to support the GRT (Gypsy, Roma, Traveller) community

To help people to manage their needs by answering their queries and supporting them in making appointments

To assist patients to be better prepared to have conversations on shared decision making and to improve awareness of shared decision making and related support tools

To liaise with other Care Coordinators in other Practices within the PCN and share best practice

To provide coordination and navigation of patients, and where appropriate their carers, across health and social care services, where appropriate working hand in hand with social prescribing link workers (SPLW)

Support the Practices on specific CQC compliance requirements

To support in the delivery of enhanced services and other service requirements on behalf of the PCN

To attend and participate in the delivery of multi-disciplinary teams (MDT) within the PCNs.

To undertake all mandatory training and induction programmes

To contribute to and embrace the spectrum of clinical governance

Pay: £27,500.00-£29,250.00 per year
