An exciting opportunity has arisen in the Joint Reconstruction and Sarcoma Scheduling Team. We are looking for a Patient Pathway Coordinator to join the team on a substantive basis. This broad, patient centred and high profile role is integral to the operational running of the department which is currently striving to meet the challenge of additional demand across the service. The main function of the role will be to coordinate and schedule the patient pathway, from referral to first outpatient appointment, through diagnostics, treatment planning, treatment, and follow-up, highlighting any potential delays in the pathway and ensuring accurate data collection to support the process.
This post is 1.0 WTE to cover Monday to Friday
The role of the Patient Pathway Coordinator is to work closely with clinicians and Service Managers to ensure that the care pathway of all patients is effectively managed.
The post holder will be required to act as a flexible administrative resource actively supporting patient pathway improvement projects across the Trust. The post holder will have direct patient contact working with the wider multidisciplinary team to ensure that patient care is coordinated and delivered with minimum level of difficulty to our patients, in the service delivery of 18 week and cancer waiting time pathways and other performance targets.
