The role of the Patient Pathway Coordinator is to work closely with clinicians and Service Manager to ensure that the care pathway of all patients is effectively managed.

The post holder will be required to act as a flexible administrative resource actively supporting patient pathway improvement projects across the Trust.

The post holder will have direct patient contact working with the wider multidisciplinary team to ensure that patient care is coordinated and delivered with minimum level of difficulty to our patients, in the service delivery of 18 week and cancer waiting time pathways and other performance targets.

Children's Hospital is looking for a Patient Pathway Coordinator to join the team on a full time, permanent contract.

This broad, patient centred and high profile role is integral to the operational running of the department which is currently striving to meet the challenge of additional demand across the service. The main function of the role will be to coordinate and schedule the patient pathway, from referral to first outpatient appointment, through diagnostics, treatment planning, treatment, and follow-up, highlighting any potential delays in the pathway and ensuring accurate data collection to support the process.

The Royal National Orthopaedic Hospital is a specialist tertiary referral hospital. A fifth of our patients are younger than 18 years of age. Patients are referred to the hospital nationally and internationally, often with complex medical needs.

**Children are admitted under different teams including**:
Paediatric Orthopaedics

Spinal Injuries

Spinal Deformity

Spinal Malformation

Bone and Soft Tissue tumours

Peripheral Nerve Injury

Paediatric Medicine (Metabolic Bone &Rheumatology)

The team consists of three band 4 patient pathway coordinators, one band 3 support and a band 5 patient pathway lead. It is a supportive team that provides cover for each other.

To be responsible for the management of the patient pathway, from referral to first outpatient appointment, through diagnostics, treatment planning, treatment, and follow-up, highlighting any potential delays in the pathway and ensuring accurate data collection to support the process.

To have a sound understanding of 18 week and cancer waiting time targets, rules and procedures, and to work proactively to ensure that all targets are maintained.

The post holder should provide a weekly update to Service Managers - so they are aware of any pathway delays or potential breaches of waiting times targets.

To routinely attend performance review meetings and ensure that agreed actions and outcomes are requested, documented and monitored.

To be responsible for booking and sending out-patient, admission / pre-assessment appointment letters and telephoning patients as required.

To schedule outpatient appointments and inpatient operating lists, ensuring that clinics and theatres are utilised appropriately and waiting lists are managed effectively.

To establish and maintain strong communication links with the clinical team, attending weekly planning meetings and ensuring lists are managed in a clinically appropriate manner as guided by the team.

To liaise with consultants and their teams to monitor and ensure correct case mix on theatre lists, making appropriate staff aware of any changes, including Site/Bed manager and Medical Records staff using the agreed protocols / shared files.

To ensure patient cancellations are allocated new TCI dates within the specified time limits.

To manage pre-assessment clinics, working in close conjunction with pre-assessment staff to ensure the smooth running of clinics.

Monitor elective surgery waiting times, proactively identifying risks, and taking corrective actions to ensure relevant targets are not breached, in conjunction with the Service teams, and clinical teams.

Deal with non-clinical queries from patients, their carers, general practitioners and members of the admin & clerical team.

Escalate potential breaches of waiting times or patient pathway bottlenecks to the Access Manager and Service Management Teams as appropriate.

To take overall responsibility for the waiting list and patient flow in conjunction with Service teams.

To monitor the quality of patient pathways using Trust IT systems and act to improve the quality of information within the Directorate.

To validate and analyse data and produce ad hoc reports as directed by the patient pathway lead.

To use hospital information systems to record and review patient information of both clinical and non-clinical nature.

To assist the Service Managers with the collection and analysis of relevant metric or performance data.
