The Integrated Transfer of Care Hub are looking to recruit a Discharge Co-ordinator to work on the wards across the acute hospital to increase patient flow and optimise patient experience surrounding all transfers of care.

We are looking for energetic and caring people who have an understanding and some knowledge of the health and social care environment to coordinate discharges and work within the multi-disciplinary team.

This is a seven day service that provides single point of contact across all wards and hospital sites within Mid Yorkshire Hospitals Trust for staff, patients, families, carers and associated people and co-ordinate/contribute to the safe and timely hospital discharge in partnership with other multi-disciplinary colleagues.
