Discharge Coordinator

Key Responsibilities To promote the correct implementation of discharge processes throughout Mid Yorkshire hospitals to ensure safe and timely discharge and smooth transfer of patients. Actively participate in the ward based discharge activity (i.e. SAFER, EDDs, board rounds and the identification of patients for transfers to PMU and DDH) to ensure that discharge activity is occurring for every patient within the ward. To maintain momentum of discharge planning throughout the entire process.
Actively work with the discharge team and hub to ensure patients are identified for transfer to Gate 11 discharge hub to ensure acute beds are available as soon as possible. Provide concise and detailed information/handover regarding patients discharge plans. Work closely with all internal and external partners and agencies to ensure a seamless service, including working to reduce delayed discharges. To link closely with clinical Site managers to ensure effective use of beds.
To be able to use problem solving skills, to determine appropriate actions and formulate specific and appropriate management plans. To have a working knowledge of current legislation, National Service Frameworks, clinical standards and national policies in order to provide an effective discharge service. Communication To maintain and promote confidentiality in line with national and Trust standards and policies. To facilitate, establish and maintain clear and effective communication networks with a variety of people, professional and external agencies.
E.g. patients, carers, multi-disciplinary team, external agencies and services etc. To communicate complex patient related information to facilitate collaborative working with other colleagues across health, social care and housing to ensure the delivery of a co-ordinated service. To effectively manage and prioritize workload, seek support and escalate if work cannot be completed in a timely manner.
To adapt approach when information may be sensitive, confidential, distressing, where conflict of opinions may occur. To inform colleagues, patients and relatives on discharge planning processes, services available and how to access them. To promote an understanding of the role of the discharge team and the discharge planning process within the hospitals and the wider community. To maintain productive working relationships within the Discharge team and ward teams using clear communication strategies.
Participate in departmental and multidisciplinary/multi agency meetings as delegated by the Ward Manager, Discharge Manager or Discharge co coordinators. Administration/ Documentation To be responsible for the written recording of up to date, accurate information in line with Trust guidelines. To abide by legal requirements governing confidentiality and data protection. To be responsible for the completion and collation of appropriate data.
To act on written information and referrals received from other members of the multidisciplinary team and outside agencies. To carry out routine administrative tasks in relation to discharge planning on the ward.
