We are excited to have created a number of novel Tier 4 (middle grade) Senior Clinical Fellow (SCF) posts to join our dynamic and friendly Emergency Medicine team, as part of a large investment expanding our medical workforce. You will pursue a special interest whilst working in Nottingham University Hospitals (NUH) Emergency Department. NUH is the East Midlands Major Trauma Centre.

Geriatric Emergency Medicine is a growing special interest within the field of Emergency Medicine, fuelled by a need to provide more holistic, individualised care for our ageing population in the context of increasing ED attendances of older patients with complex needs.

This role would be an ideal post for someone looking to improve their skills and knowledge in managing complex older people with frailty and to gain experience in collaborative working across patient pathways relating to older people. In this post, you will spend 60% of your time undertaking a multitude of supervised learning opportunities in a variety of settings, managing older people with frailty. There will also be an opportunity to receive funding for the Diploma of Geriatric Medicine and attend local teaching provided for our geriatric trainees.

**Main duties of the job**:
This is a full time post in Emergency Medicine, in which:

- 60% of your time time will be spent pursuing your interest in Geriatric Emergency Medicine in a variety of settings and working with our departmental frailty team and colleagues in the Trust and across the Integrated Care System to improve the care of older people in ED and across the emergency pathway.
- 40% will be clinical, on the Tier 4 registrar rota providing cover in all the different areas of our ED (Majors Unit, Resuscitation Area, Urgent Treatment Unit, and the Paediatric Emergency Department

You will also be expected to take a leadership role in our Healthcare of Older People (HCOP) Interface Collaborative to improve patient pathways in this area, as well as supporting quality improvement, research and educational innovations

Full/ limited registration with the GMC is essential. Applicants will have passed the MRCEM (or equivalent) and be competent to work to Tier 4 (middle grade) level with at least six months’ experience in Emergency Medicine at registrar level. You will ideally have at least 3 months’ experience in anaesthesia (including completion of Initial Anaesthetic Competency certificate) and experience of critical care is desirable. Prior experience of working in the NHS or a comparable healthcare system is essential

There are multiple inpatient, outpatient and community services at NUH that will provide opportunities for the appointee to develop expertise in comprehensive geriatric assessments (CGA), including working on the Frailty admissions ward which incorporates a short stay unit (D57 and OPAU), supporting and working with the acute frailty team located in ED, gaining experience in a dedicated Medical and Mental Health Unit for older people with mental health issues and delirium.

The successful applicant will have ample opportunity to develop their skills in contributing to CGA in patients with multiple co-morbidities, polypharmacy and functional limitations, within a supportive multidisciplinary team.

Our Emergency Department

Our forward thinking, research active Emergency Department is one of the busiest in Europe with 180,000 new attendances annually, with 44,000 children and young people seen in our Children’s ED.

The 9 bayed Resuscitation Area includes 2 adult and 1 paediatric major trauma bays, and the Majors Area has 30 monitored bays. The Urgent Treatment Unit (UTU) is a co-located area for ED and Primary Care assessment of ambulatory illness and injury patients. The expansion and rebuild of our new children’s ED is currently underway and includes a new purpose built paediatric resuscitation area.

The post is open to all applicants with a specialist interest in Geriatric Emergency Medicine, who meet the essential criteria detailed in the person specification and wish to gain further experience in the management of a wide range of medical and surgical conditions presenting to all areas of the Emergency Department.

The post will offer the opportunity to gain experience, skills and knowledge to support management of older people in Emergency Medicine, following an established curriculum developed by leaders in the field of Geriatric Emergency Medicine and Acute Frailty, which can be adapted to individual requirements.
