Band 6 Care Coordinator

Bradford, Bradford, United Kingdom Bradford District Care NHS Foundation Trust Full time

An exciting opportunity has arisen for a Band 6 Community Care Coordinator to work in our lively and friendly team based in the multi-cultural city of Bradford.

We are looking for skilled, enthusiastic, and passionate clinicians to join our fantastic City and North Community Mental Health Team for Older People.

We enjoy working in one of the most diverse and culturally rich populations in England and are always looking for new and innovative ways of delivering care.

Working with our older population is a privilege.

It provides unique opportunities to affect overall quality of life for a population that can be predisposed to comorbidities, reducing physical health, reducing mobility, and isolation.

Working with people in their most challenging of times, you are helping to mentor them to become the best possible version of themselves.

As a band 6 clinician you will be expected to act as a role model and resource for our less experienced staff.

You will also have opportunity to be involved in service delivery improvements and innovations.

You should be caring, flexible, and creative. Able to thrive under pressure, you will know how to connect with people on all levels.

You will be committed to the delivery of high-quality service, working both independently and as part of an established team.

You will act as a role model and resource for our less experienced staff and students and will also have opportunities to be involved in service delivery improvements.

As a Community Care Coordinator, you are not only supporting clients through what can often be the most challenging of times, you are also mentoring them to become the best version of themselves.

Having an open mind and a flexible approach to care is key.

This helps to establish the best possible ways to care for your clients within a recovery-focussed mindset, ensuring that you never lose the motivation that change is achievable for all.

All care that you deliver will be in line with current care pathway frameworks and national guidelines.

Working within our multidisciplinary team, you will enjoy being part of an organisation that relies upon the professionalism, insight, expertise, and passion of its staff to inspire individual recovery for the people we work with, whilst also nurturing, developing, and encouraging our staff to achieve their career aspirations.

You will be offered regular supervision and support.

This is truly an exhilarating time to join our team.

As we embark upon an exciting transformation project, you will not only be an important part of our new journey, but equally will contribute heavily towards it.

Successful applicants will be able to demonstrate a high level of commitment, motivation, and reliability. You will have good awareness of current health and social care issues, alongside excellent time management skills. Due to the community element, you will hold a full and valid driving licence.

Everything we do is underpinned by our core values:

  • We Care
  • We act with respect and empathy, and always value difference
We Listen

  • We understand people's views and respond to their individual needs
We Deliver

  • We develop and provide excellent services and support our partners
We will consider a variety of flexible working arrangements to enable staff to balance their work and home lives and support staff by offering a fantastic range of benefits including:
Increase mileage rates for > 3500

A wide range of health and wellbeing support packages

A range of internal and external development and learning opportunities

Plus many more.

We are also proud to pledge our support to the Armed Forces and hold a Bronze Award in the Defence Employer Recognition Scheme.

BDCFT is dedicated to environmental and social sustainability by delivering on the commitments within our Green Plan.

All staff are actively encouraged and supported to participate in training and to identify and implement sustainable quality improvement across all service areas and activities.

Please read the attached Job description and person specification for more information about this role.
