Job Description:
Temporary Band 5 Registered Mental Health Nurse (RMN):

Bradford District Care NHS Foundation Trust is currently seeking a skilled and dedicated Band 5 Registered Mental Health Nurse (RMN) to join our team at Lynfield Mount Hospital in Bradford, West Yorkshire.

This is a temporary position with a start date of 19/02/2024 and an end date of 20/02/2024.


As a Band 5 RMN, you will play a crucial role in providing high-quality care and support to individuals with mental health conditions.

As a Band 5 RMN, your responsibilities will include:

1.

Assessing and Planning:
You will conduct comprehensive assessments of patients' mental health conditions and develop individualized care plans. This will involve collaborating with multidisciplinary teams and using evidence-based practices to ensure the best possible outcomes for patients.

2.

Delivering Care and Treatment:
You will provide direct care and treatment to patients, administering medications, facilitating therapeutic interventions, and implementing care plans. You will also monitor patients' progress, evaluate the effectiveness of interventions, and make necessary adjustments to ensure optimal outcomes.

3.

Promoting Recovery and Wellbeing:

As a Band 5 RMN, you will support patients in their recovery journey, fostering a therapeutic environment that promotes their mental health and wellbeing.

You will empower patients to develop coping strategies, provide psychoeducation, and promote self-management of mental health conditions.

4.

Safeguarding and Risk Management:
You will be responsible for identifying and managing any potential risks to patients' safety and wellbeing.

This includes implementing safeguarding measures, conducting risk assessments, and collaborating with relevant agencies to ensure the appropriate level of support and protection for vulnerable individuals.


5.

Documentation and Record Keeping:
Accurate and timely documentation is a crucial aspect of the Band 5 RMN role. You will maintain comprehensive patient records, documenting assessments, care plans, interventions, and progress. This will facilitate effective communication and continuity of care within the multidisciplinary team.

6.

Collaboration and Communication:

As a Band 5 RMN, you will collaborate with other healthcare professionals, patients' families, and external agencies to coordinate care and support.

Effective communication skills, both verbal and written, are essential for building therapeutic relationships and ensuring the holistic care of patients.
