You will be expected to work proactively & creatively as part of a multidisciplinary team to deliver high quality, evidence basednursing caretoadults & young people with a learning disabilityin partnership with them & their families/carers.

You will have the opportunity to work closely with other colleagues in the multi-disciplinary team, and willprovidespecialist advice, support & training to those on your caseload, theirfamilies& carers, and to your colleagues across the Health board.

Youwill also be supported to develop your own clinical competency profile & to develop skills in specialist areas of practice.

Applicants should have postgraduate experience of working as a Registered Learning Disability Nurse within a healthcare setting, have a broad knowledge ofclinical specialties, with evidence of Continuing Professional offer regular supervision, professional development& access to peer Nursingsupport.

Various full and part time hours are available.

Main duties of the job

We are looking for a suitably qualified enthusiastic & motivated nurse with current NMC registration to joinour Learning Disability Community Team based in Carmarthen.

You will have continuing professional accountability and responsibility for the provision of specialist health care, treatment and advice for a defined caseload of clients who have learning disabilities with complex health needs, and their carers, in the community.

Will liaise and work in partnership with other agencies to deliver a flexible and responsive service which meets the need of the local population of people with a learning disability and the needs of carers.

Will provide specialist advice, support and training on a range of health issues to other agencies who are responsible for the provision of services/care to people with learning disabilities with the aim of reducing health inequalities for people with a learning disability.

We would expect any interested applicants to be up to date with current policy/legislation & demonstrate an ability to improve service delivery by promoting change inhealth relatedprojects.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Qualifications and Knowledge

Essential

Registered Nurse Learning Disabilities (RNLD) with a current NMC registration. Post-registration training with evidence of ongoing professional development in the field of learning disabilities Evidence of Post-graduate Certificate or Diploma or working towards or equivalent clinical experience Comprehensive understanding of current policy and practice in relation to people with learning disabilities. Teaching qualification or equivalent, or recent experience in the provision of training to health staff. Evidence of risk assessment and risk management skills.

Desirable

Comprehensive understanding of local and national drivers for change in the field of learning disabilities.

Experience

Essential

Extensive experience of working with people with learning disabilities at Band 5 or above. Experience in the assessment and management of people living with co-morbidities and complex health needs. Recent experience of working within a Community Team at Band 5, or extensive experience of working within a multi-disciplinary team. Recent experience of supervising non registered support staff. Evidence of working with people who display behaviours that challenge.
