Looking for a role where you will be working withsome of the city's most vulnerable people. With the Frailty team your role will primarilybe providing urgent assessments to support the team in preventing unnecessaryadmissions to hospital. Providing short term interventions that will helpsupport maintaining safety and independence at home. You will also have the opportunityto work in other areas including reablement and expediting early dischargesfrom hospital.

You will be working as part of an expanding FrailtyHUB which has and anticipatory, crisis and discharge arms. We integrate withother services, both primary and secondary care , social services , CVS andYAS.

Main duties of the job

To complete physiotherapy assessments to in thecommunity preventing admission to hospital and providing communityrehabilitation. This will include treatment planning, assessing risk,promoting, and enabling independent functioning.

To provide training, supervision and advice totherapy assistants, and other therapists/ nurses in the team

To participate in the planning, development, andevaluation of community therapy service, holding responsibility for definedprojects.

To maintain close working relationships with otherhealth professionals /referrers to ensure efficient, effective, and appropriatedelivery of the service.

A full driving licence and access to a vehicle thatcan be used for work is essential.

Job description

Job responsibilities

1. To contribute to developingappropriate clinical networks which may be at a local, regional or nationallevel that improves health and social care outcomes for patients and encouragesreduced hospital admissions.

(i) To contribute to appropriateclinical networks which may be at a local, regional or national level.

(ii) To Contribute to NimbuscareValues

2. To integrate, evaluate andcontinuously develop physiotherapy services within the Frailty Team that improves health and social careoutcomes for patients and encourages reduced hospital admissions.

(i) To contribute to thedevelopment of the Physiotherapist role within York Integrated Community Teamand expansion of services.

(ii) To be responsible for adefined area of service working without direct supervision. Clinical work isnot routinely evaluated; however clinical work will be evaluated in line withPrimary Care and service objectives. Access and advice are available fromsenior professional colleagues if required.

(iii) Initiate evidence-basedaudit and projects to evaluate and improve clinical practice. Initiate andevaluate user involvement and where appropriate make local service changes.

(iv) To provide support,specialist advice and training to more junior members of staff and students. Toact as a resource providing information and advice regarding patients.

(v) To ensure a holisticapproach to the overall care of patients including effective workingrelationships with MDT, wheelchair and specialist equipment providers,consultants, and relevant others.

3. Provide competent,professional, and autonomous specialist physiotherapy services to patients andcolleagues within the Frailty Team and encourages reduced hospitaladmissions/expediate hospital discharges .

(i) To provide professional,competent and efficient care which meets patients' health and social needs andthat promotes a reduction in the number of inappropriate hospital admissions.

(ii) To undertake the specialistPhysiotherapy assessment and management, of a designated group of patients,within the specialty of patients within the Frailty Team. This will involveworking with individuals, groups and parents / carers and relevant others,providing specialist assessment and treatment in the most appropriateenvironment.

(iii) To undertake acomprehensive assessment of patients, including those with diverse or complex/multi-pathology presentations. Specialist functional assessment of cognitive,perceptual, sensory and motor skills, which may include the use of standardisedtesting. Use advanced clinical reasoning skills and assessment techniques, toprovide an accurate treatment plan and to evaluate any intervention.

(iv) To formulate and deliver aspecialist, individual, goal orientated treatment programme based on soundknowledge of evidence-based practice and treatment options using assessment,clinical reasoning skills and knowledge of treatment , treatment techniquesand principles, equipment provision and other alternative options.

(v) To determine clinical needand intervention indicated. To work as an autonomous practitioner, holdingresponsibility for own caseload of patients with complex physical,psychological, and cognitive problems.

(vi) To provide training andadvice on changes and adaptations to the patient's lifestyle considering theenvironmental, emotional, physical, spiritual, and social needs. To address andprovide comprehensive diagnostic and treatment to support patients' mobilityperformance and skills deficits enabling the patient in areas ofself-maintenance, productivity, education, and leisure.

(vii) Provide support to othercolleagues when dealing with this group of patients.

(viii) To work flexibly as amember of the department responding to relevant service needs

(ix) To maintain records as anautonomous practitioner in accordance with legislation, professional andservice policy. To contribute to the collection of business related informationin order to inform service development and priorities.

(xi) To be responsible fororganising and planning own caseload, to meet service and patient priorities.Readjusting plans as situations change/arise. Delegate duties to other membersof staff as appropriate.

Key Responsibilities include:

Motivate and encourage patientsto participate in treatment programmes and where necessary alter treatment tomaximise potential. To include families, carers, and support staff in treatmentprogrammes.

To use a range of verbal andnon-verbal communication tools to communicate effectively with patients andrelevant others. The caseload will include individuals who have difficulties inunderstanding or communicating.

To work within the clinicalguidelines of the Chartered Society of Physiotherapy and HCPC standards and tohave a working knowledge of national and local standards and monitor ownquality and safety of practice.

To undertake the measurement andevaluation of your work and current practices through the use of evidence basedpractice projects, audit, and outcome measures individually or with colleagues.

To provide clinical supervisionand support to colleagues as required. To supervise and educate junior staffwithin the workplace.

On a daily basis to beresponsible for the safe and competent use of appropriate specialist equipment,patient appliances and aids. Also to provide teaching on the use of equipmentto the patient, carers and other agencies involved in the care. This alsoinvolves the assessment and provision of appropriate specialist equipment topatients including specialist knowledge of wheelchairs, seating and skillstraining in the use of indoor powered chairs, to maintain appropriate verbaland written communication associated with this. You will manage unpredictableclinical situations in a community setting where access to specialised supportmay be limited.

To maintain and developknowledge of current evidenced-based practice and develop specialist knowledgeof particular conditions and patient types within the specialty.

To be an active member of thein-service training programme by attendance at, and participation in, anddelivery of in-service training programmes, tutorials, individual trainingsessions, external courses and peer review.

To undertake ass directed thecollection of data for use in service evaluation, audit and research projects.
