In this role, you will engage with some of the most vulnerable populations in our area. As part of the Frailty Team, your primary responsibility will be to conduct urgent assessments aimed at preventing unnecessary hospital admissions. You will provide short-term interventions that promote safety and independence at home. Additionally, you will have the chance to work in other sectors, including reablement and facilitating early hospital discharges.

You will be an integral member of an expanding Frailty HUB, which encompasses anticipatory, crisis, and discharge services. Our approach integrates various services, including primary and secondary care, social services, and community voluntary services.

Main Responsibilities

Your key responsibilities will include:

  1. Conducting physiotherapy assessments in the community to prevent hospital admissions and deliver community rehabilitation. This involves treatment planning, risk assessment, and promoting independent functioning.
  2. Providing training, supervision, and guidance to therapy assistants and other team members.
  3. Participating in the planning, development, and evaluation of community therapy services, taking charge of specific projects.
  4. Maintaining strong collaborative relationships with other healthcare professionals and referrers to ensure effective service delivery.

A valid driving license and access to a vehicle for work purposes are essential.
