In this role, you will engage with some of the most vulnerable members of our community. As part of the Frailty Team, your primary responsibility will be to conduct urgent assessments aimed at preventing unnecessary hospital admissions. You will provide short-term interventions that support individuals in maintaining their safety and independence at home. Additionally, you will have the chance to work in other areas, including reablement and facilitating early hospital discharges.

As a member of our expanding Frailty HUB, you will be involved in anticipatory care, crisis management, and discharge planning. Our team collaborates with various services, including primary and secondary care, social services, and community organizations.

Main Responsibilities

Your key responsibilities will include:

  1. Conducting physiotherapy assessments in the community to prevent hospital admissions and facilitate rehabilitation.
  2. Providing training, supervision, and guidance to therapy assistants and other team members.
  3. Participating in the planning, development, and evaluation of community therapy services, taking charge of specific projects.
  4. Maintaining strong working relationships with other healthcare professionals to ensure effective service delivery.

A valid driving license and access to a vehicle for work purposes are essential.

Key Qualifications

Essential qualifications include:

  • Registered and ongoing membership with the Health and Care Professions Council (HCPC), ensuring up-to-date professional skills and knowledge.
  • Relevant professional physiotherapy qualification.
  • Enhanced Criminal Record Check certification with children and/or adults barred list check(s).

Desirable qualifications include:

  • Safeguarding Children Level 2 Training.
  • Safeguarding Vulnerable Adults Training.
