We are seeking Generic Support Workers to join either York or Selby Community Response Teams (applicants will be able to offer a preference of base / locality at interview) - to deliver high standards of multi-disciplinary interventions to patients within their own place of residence to prevent avoidable admissions to acute care and to support discharges.

This role will be delivering interventions which support activities of daily living and promoting independence, as able.

Our well-established teams cover the historical city of York, market town of Selby as well as the rolling countryside within North Yorkshire. The team consists of Physiotherapists, Occupational Therapists, Nurses and Support Workers and works alongside Primary Care and Partners to provide Urgent Care Response, Frailty Hub as well as short term reablement/rehabilitation programmes.

You will undertake duties delegated by the Nursing, Physiotherapy and Occupational Therapy teams, and will be responsible for providing holistic, person centred, compassionate health care to patient’s 18 years and over, supporting rehabilitation, reablement and recovery within the comfort of the patients home.

The ability to transport yourself and equipment across the locality during the course of a normal working day is required for this vacancy.
