Job summary

TheSandwell Community Dementia Support Service (SCDSS) is an innovative servicecommissioned by Sandwell Metropolitan Council and the Black Country and WestBirmingham Clinical Commissioning Group. As lead provider, we are workingalongside seven other local organisations in a dynamic partnership with AgewellCIC, Dementia Pathfinders CIC, West Bromwich African Caribbean Resource Centre,St Albans Community Association, BUDS, Age UK Sandwell and Alzheimers Societyto provide this service across Sandwell Metropolitan Borough.

Main duties of the job

We arerecruiting a Service Manager to support people affected by dementia orexperiencing memory loss, to ensure they have the appropriate and timely helpthey need.

The Service Manager will lead a team of town-based Dementia Navigators and Advisers, working closely with the Operations and Service Development Executive on developing the service and sustaining strong strategic partnerships with statutory and community organisations throughout Sandwell, working closely with Third Sector and Community Groups.

We are looking for an experienced manager with the ability to develop effective team working and have proven leadership and service delivery skills along with a passion for supporting people living with dementia and memory loss.

Job responsibilities

Leadership and ServiceDevelopment:

Work strategically with the ExecutiveTeam and Provider Collaborative Partnership, statutory and third sectororganisations, commissioners and PCNs to integrate the service pathway withinthe health and social care system.

To promote and encourage service-user involvementand co-production in all aspects of the service.

To support Provider Collaborative partners toidentify gaps in provision in the health and care system, flagging up these gapsto senior managers and commissioners by means of agreed communication channels.

To develop robust and strong relationships withethnic minority groups and people with protected characteristics, includinginformal family carers, within each of the Sandwell six towns, embracing aninclusive approach to developing resilient and strong communities.

To work with the Operations and DevelopmentExecutive and partnership senior managers to deliver the service contract KPIs.

Develop partnerships with PCN/GP leads, MentalHealth Trust Adult Social Care and acute services leads to promote SCDS,evaluating the effectiveness of the service reach and integration of delivery.

Operational responsibilities:

To manage the service asa whole working closely with the partnership team to deliver the full-servicespecification.

To support the policy,procedure and practice for all staff employed within this service and supportlead partners and their staff, taking a whole system approach.

To work in conjunctionwith all partners ensuring there is clear and good communication with internalcolleagues, partners and service team members to deliver a quality service andto meet contractual requirements.

To be responsible forholding regular meetings with team and partners to ensure contract complianceKPI returns are returned by the required deadlines.

To create effective channel of communication withinternal & external stakeholders, including Black Country Healthcare NHSFoundation Trust, Sandwell and West Birmingham NHS Trust, iCares, STARS,Sandwell Metropolitan Council, GP Practices/PCNs.

To provide accurate KPIreporting and analysis of data to senior managers and commissioners to meet thecompliance requirements for this contract.

To ensure the CRM system is capturing the relevantdata to support KPI evidence.

To providemanagement guidance and support (with partner organisations) and clinicalsupervision for Dementia Navigators and Community Dementia Advisers, tosafeguard the wellbeing of staff and address development needs of the team.

To support trainingarrangements to ensure skills set and knowledge of staff working within thisservice. Making sure screening tools and procedures are fully understood, andstaff have the knowledge and understanding to support service users with legal,financial issues to include LPAs and Advance Care Planning.

To check staff compliance with the use of CharityLog CRM software system to ensure accuracy and timely recording to meetcontract activity reporting.

To be a designated safeguarding lead, responsiblefor managing and embedding safeguarding practice within the partnership,ensuring internal and local safeguarding procedures are followed, and addressing identified issues toensure vulnerable people are protected and harm is prevented.

To work with theOperations and Development Executive and service users in the co-development ifthe service - ensuring we have focus groups and events, as well as regularclient feedback, regular client surveys and focus groups.

Marketing and Promotion:

To promote the service as required by theOperations and Development Executive, presenting the service to Health andSocial Care professionals.

To ensure that themarketing and promotion of the service functionality is supported and embeddedwithin the collaborative partnership.

To ensure that there is astandard message that is easily understood and enables all potential serviceusers and those likely to refer into the service to understand the pathway andhow people with a memory problem will benefit from being supported by theservice.

To lead on thedevelopment of raising the awareness of Dementia Friendly Communities to thecommunity, education settings and other organisations within Sandwell.

To actively help andsupport leads within each of the six towns to have local resources and serviceawareness to provide appropriate access to information and advice to provide toservice users, building knowledge and resources and increasing awareness ofdementia within local organisations to support service users through theirjourney of dementia.

Proven Leadership ability in a Health, Primary and Social Care environment. Demonstrable experience of working proactively in a busy environment and developing inter-professional relationships. Experience of CRM systems and database management.

Desirable

Experience of health and social care contract management and delivery, and good understanding of health and social care funding arrangements. Experience of working with family carers. Experience of co-production and community development work.
