Job summary
Central London Community Healthcare NHS Trust provides specialist community services to children and adults with sickle cell disease and thalassaemia. Following investment from NHS England to improve Sickle Cell Community Services in North West London, CLCH are recruiting to 3 new posts, including Community Children's Haemoglobinopathy Nurse . This expanded service will provide nursing care to adults with Sickle Cell in the boroughs of Ealing, Hillingdon and Hounslow, in addition to our current service in Westminster, Kensington & Chelsea and Hammersmith & Fulham.
We are seeking a community nurse, with a background in children's nursing, who is interested in gaining experience of nursing children with haemoglobinopathy conditions and/or genetic counselling in Westminster, Kensington & Chelsea and Hammersmith & Fulham and Hillingdon. Training will be provided to ensure the knowledge and skills to do the role. The service works closely with the specialist team at the Brent Sickle Cell Service, who provide community nursing services to the boroughs of Brent and Harrow, and clinical supervision for Haemoglobinopathy across NWL ICB.
Main duties of the job
You will work alongside the current Specialist Children's Nurse, managing the caseload of children living with Haemoglobinopathy conditions within the inner boroughs of Westminster, Kensington & Chelsea and Hammersmith & Fulham, and working with the wider team in offering clinical support to people with haemoglobinopathy conditions at different stages of their lives, from antenatal screening, throughout childhood, through to adults living with a long term chronic condition. A new pathway of preconception counselling will be developed as part of this investment.
Your office base will be Woodfield Road Medical Centre. Patients will be supported by the team at Imperial NHS Trust sites of Hammersmith and St. Marys Hospitals, and the team will provide nursing support in patient homes and other venues local to their homes across the 6 boroughs.

Job description
Job responsibilities
To support a community-based clinic for clients and at risk communities requiring/requesting testing in order to identify those with haemoglobinopathy, including the provision of a phlebotomy service, which offers counselling and partner testing for women prior to or in early stages of pregnancy.
To follow up all those identified with a significant haemoglobinopathy and provide appropriate interventions for children, young people and in transition to adulthood, as part of life long support.
To respond to positive haemoglobinopathy results of new born babies in the inner boroughs of CLCH.
To provide home visits to newly diagnosed babies with significant haemoglobinopathies within a designated period and liaise with the Health Visitor.
To provide continuity of care for the families of infants identified with major haemoglobinopathy, ensuring that they receive the appropriate package of care.
To be responsible for ensuring appropriate paediatric shared care is initiated for all affected babies, and follow up arrangements are in place, leading the communication between nominated paediatrician, GP, Health Visitor and specialist referral centre.
