Job summary

An exciting opportunity has arisen for an experienced physiotherapist with a specialist interest in Frailty and Older Peoples Medicine to join the dynamic Frailty Physiotherapy team at Worcestershire Acute Hospitals NHS Trust.

The frailty service is a priority for the trust and as such the service is being invested in and is under-going exciting positive changes to enhance the services provided.

This is a permanent full-time post ( hours) and will involve working across sites at Worcester Royal Hospital and the Alexandra Hospital, Redditch.

You will be clinically responsible for providing a timely, evidence-based service to older people living with frailty, working effectively as part of the specialist Frailty MDT to optimise outcomes.

The service includes the Geriatric Emergency Medicine service, the Acute Frailty Unit at Worcestershire Royal Hospital, and the inpatient frailty wards at the Alexandra Hospital, Redditch.

You will have the opportunity to develop your leadership and management skills, working closely with senior members of the team in managing staffing levels and caseload to ensure equitable provision of physiotherapy across the service.

As the service evolves, you will be expected to participate in the multidisciplinary 7-day service for frailty.

There is also an expectation that you participate in respiratory out-of-hours and weekend services.

Main duties of the job

Workingas an autonomous practitioner, to provide high quality patient focussed carethrough assessment, diagnosis, development and delivery of individual treatmentprogrammes. Totake the responsibility for own caseload within a defined area of service orfor a particular patient type. Toprovide supervision, support and education to qualified and unqualified staffand students. To participate in departmental audit insupport of evidence based practice. To communicate with the MDT asappropriate to provide comprehensive assessment to determine admissionprevention as appropriate to given speciality. To undertake outpatient activity asappropriate within area of speciality. To contribute to our 7-day service on arota basis dependant on specialty of work. To complete on call over a 24 hrs period/ complete weekend working as appropriate to speciality.

Key Duties

Clinical:

Tobe professionally and legally accountable for all aspects of practice,including the management of patients receiving treatment. Toundertake clinical assessment, including risk analysis, of patients presentingwith simple and complex/multi pathologies within defined area of speciality. Tobe able to apply knowledge and highly developed skills from a wide range ofspecialties respiratory, neurology, MSK out patients, care of the elderly dependant on given areaof speciality underpinned by evidence. Touse advanced clinical reasoning skills and manual assessment techniques toestablish accurate diagnosis. To usejudgemental skills and comprehensive knowledge of physiotherapy treatments toformulate individual treatment programmes based on evidence of best practice. Todemonstrate a high level of skill indexterity, co ordination and sensory skills to deliver appropriatetreatments selected from a range ofphysiotherapeutic interventions normal movement, motor relearning, manualtherapy techniques, respiratory techniques, use of NIV, exercise therapy,hydrotherapy and electrotherapy dependant on area of speciality. Toregularly evaluate the effectiveness of the treatment plan and adjust thetreatment accordingly. Touse clinical knowledge to formulate a prognosis and recommend an appropriatecourse of action and discharge plan. Tomaintain comprehensive, accurate and contemporaneous treatment recordsincluding valid consent appropriate to the intervention. Toensure that patient confidentiality is maintained at all times. Tobe an active participant on the evening and weekend On Call Service, whichincludes advanced respiratory techniques and non-invasive ventilatory aids.This may also include the treatment of children. Rota will depend on Speciality Tobe responsible for leading patient classes, cardiac rehabilitation, kneegroup and rheumatology classes dependant on area of speciality

Communication:

Toassess capacity, gain valid consent and to work within a legal framework withpatients who lack capacity to consent to treatment. Tocommunicate effectively, verbally and through touch, with patients to maximiserehabilitation potential. Todemonstrate communication skills of persuasion, motivation, reassurance,explanation and negotiation, with patients with barriers to effectivecommunication may be evident, eg expressive and receptive dysphasia, alteredperception, hearing loss, mental disabilities, pain and fear. Barriers to effective communication willregularly be evident. Towrite clear and concise notes based on physiotherapy assessment and treatmentof patients, in accordance with professional standards. Touse written information in assisting the patient to understand their conditionand involvement in the treatment process. Toliaise with the designated Team Lead Physiotherapist regarding the workload ofthe area. Toliaise with senior physiotherapy staff regarding highly complex cases. Toliaise with medical, nursing staff and other health care professionalsregarding patient care. Toliaise and where appropriate advise patients, carers and relatives regardingpatient care. Toliaise with members of the multidisciplinary team and attend case conferences,MDT meetings and ward rounds as appropriate. Toproduce written discharge summaries and/or progress reports to GPs, Consultantsor other referring health professional regarding the patients condition withinSpeciality.

Educationand Training:

Tobe appraised by the Team Lead Physiotherapist within area of speciality, agree personalobjectives to be achieved within area of speciality and formulate a personaldevelopment plan. Tomaintain own clinical professional development by a variety of means(internal/external courses, journal reviews, self-directed study) in order to maintainup to date clinical knowledge. Tokeep abreast of new developments in clinical physiotherapy and evidence basedpractice. Toparticipate in all annual mandatory training. Todemonstrate an understanding of clinical governance in physiotherapy practice.To regularly evaluate physiotherapy practice through the use of outcomemeasures and audit. Toundertake, or assist with Research and Development activity. Toparticipate in training by the regular delivery of presentations to fellowphysiotherapists, other staff and students. Tosupervise and appraise more junior members of staff, assistants and students asdelegated by senior staff, to promote a high standard of patient care. Toadhere to local department and trust policy.

Organisation and Management:

Toprioritise and manage own patient workload and that of supervised staff. Inthe absence of the Team Lead Physiotherapist in the specialty, to manage andallocate staff in order to provide an efficient service. Toensure the Team Lead is fully informed of all matters affecting the specialityby regular reporting. Tobe aware of and adhere to National, Professional, Trust and Departmentalpolicies and recommend changes for own work area following audit or research. Tomaintain a safe environment at all times for patients, visitors and staff. Toensure safe use of all mechanical and electrical equipment or orthoses andappliances used in the care of patients, providing advice to other staff andcarers as needed. Toact immediately to rectify any unsafe practices or environments witnessed andreport adverse incidents or near misses according to Trust policy. Toprovide statistical information regarding own workload as required formanagerial purposes
