Worcester Acute Hospitals NHS Trust is looking for an highly motivated, enthusiastic and forward thinking Nurse or Practitioner Consultant within Frailty (NMC/HCPC/GPhC), you wont want to miss out on this opportunity

As a Nurse Consultant or Practitioner Consultant, you will play a crucial role in supporting a team of Trainee and qualified ACPs. Your primary focus will be establishing an integrated frailty pathway across organisational boundaries to enable the system to provide outstanding care for our frail patients.

Job description

Job responsibilities

Plans and manages own workload and the workload of the service, in a flexible manner, ensuring that service provision meets the needs of the patients.

Provides a visible clinical profile as a Consultant practitioner, having leadership,advisory and clinical input into quality and harm-free patient care.

Carries out highly complex patient assessment, using expert clinical acumen

Independently prescribes medication within legal, national and local frameworks considering efficacy, safety and cost from the formulary.

Identification of new therapeutic interventions including drugs.

Autonomously develops, implements and evaluates patient management plans for new and reviewed patients, using advanced clinical reasoning and decision-making skills, often without guidelines, in line with best practice guidance and evidence-based standards of care.

Initiates and accepts appropriate and timely consultation and/or referral of patients.

Provide on-going follow-up in conjunctionwith other senior clinicians through ambulatory care settings.

Carries out a defined range of diagnostic tests and specialist treatments, which are identified as advanced clinical skills, using highly developed neurological and physical assessment skills.

Assesses and intervenes to assist the patient in urgent and emergency situations, in collaboration with the multi-disciplinary team.

Rapidly and continuously evaluates the patients changing condition and response to therapeutic interventions and modifies the plan of care or optimal patient outcomes.

Plans and manages complete episodes of care, working in partnership with others, delegating and referring as appropriate to optimise health outcomes and resource use.

Makes appropriate onward referrals for specialist input.

Acts as a role model, to promote consistently high standards of professional practice. To produce and promote evidence-based practice to enhance patient care.

Acts as clinical expert and lead in Frailty care, practising at an advanced level, demonstrating the ability to make critical judgements regarding patient care pathways, drawing on highly developed specialist theoretical, research and advanced practical knowledge within Frailty.

Analyses and interprets patient history and examination, presenting symptoms, physical findings and diagnostic information, often of a highly complex nature.

Orders and interprets specialist screening and diagnostic tests, based on expert knowledge and experience to aide diagnosis if appropriate.

Refers to appropriate senior clinicians when the diagnosis of conditions are questionable.

Uses sound judgement and analytical skills in directing and delivering care, assessing conflicting priorities and needs, demonstrating a highly advanced level of critical thinking and diagnostic reasoning skills in clinical decision-making.

Exercises independent clinical judgement by analysing and interpreting clinical signs and considers the range of options available.

Agrees on consensus plan when expert opinion differs or when the research evidence is not available.

Provides expert advice to healthcare professionals, both within the Trust and wider community, on clinical management of their patients.

Participates in internal and external working groups, services and initiatives to provide advice and expertise and to share best practice.

Works in partnership with other senior clinicians, staff and patients to co-design and develop person centred services to improve the patient experience.

Uses their professional judgement in managing complex and unpredictable care events and captures the learning from these experiences to improve patient care and service delivery.

Communications:

Uses highly developed communication skills to discuss or impart sensitive diagnoses or permanent severe disability. These can often be highly emotive, regularly dealing with emotionally distressed patients and relatives.

Demonstrates highly developed communication skills to provide guidance, advice, support and therapeutic interventions to the patient regarding management of their health / illness condition and its impact on patients / carers life.

Provide highly complex, sensitive information to patients and their families / carers, to support an empower them to make health care choices.

Identifies and overcomes barriers to patient understanding, providing support and identifies appropriate therapeutic interventions whilst communicating highly complex, sensitive and sometimes distressing information.

Communicate effectively, verbally and through touch with patients. Communication skills of explanation, motivation and being realistic are used with a variety of patients. Assess capacity, gain valid consent and work within a legal framework with patients who lack the capacity to consent to treatment when prescribing or discussing future care options or when randomising patients within trials.

As part of the Frailty Senior Team, takes a key role in the formulation of long term strategic plans which impact across the service design of patient pathway across the health economy.

Constantly seek opportunities to innovate and drive improvement within their area of responsibility, leading their staff in the use of service improvement methodologies, in particular measurement for improvement.

Participation in the development, implementation, monitoring and evaluation of evidence based policies / guidelines / protocols / pathways relevant to Frailty with focus on quality, safety and patient experience.

Assesses risks to health, safety and security using the results to take appropriate action to promote and improve practice.

Contributes to the clinical governance process within the division.

Works across professional, organisational and system boundaries, proactively developing and sustaining new partnerships and networks to influence and improve health, outcomes and healthcare delivery systems.

Explores opportunities for local, national and international developments in specialty, research and advanced practice, through professional and patient organisations.

Identifies areas where investment should be made to further develop the service and feed this into the business planning cycle locally and nationally.

To explore research and practice development opportunitieswithin Frailty services and provide direction and support for staff to engage with the research process.

To be an active member of the in-service training programme by leading, participating and disseminating information to relevant staff.

Have systems in place for the identification of poor practice and monitoring of development plans for all staff in liaison with the lead practitioner / matron.

Essential

Relevant post graduate qualification demonstrates theoretical and practical knowledge in Frailty. Masters Level Degree Non-medical independent and supplementary prescriber. Recognised post-graduate teaching qualification. IRMER regulation course

Desirable

Project / business management course. Commitment to studying for a relevant doctorate. Advanced communication / counselling skills.

Demonstrate leadership and nursing experience at senior level in acute and Frailty care . Working in a multi-disciplinary team. Teaching and mentoring experience.
