Position Overview
Worcestershire Acute Hospitals NHS Trust is seeking a dedicated and proactive Nurse or Practitioner Consultant specializing in Frailty (NMC/HCPC/GPhC). This role is pivotal in enhancing the care pathway for our frail patients across various healthcare settings.
Key Responsibilities
As a Consultant Practitioner, you will:
- Oversee and manage your own workload and that of the service, ensuring patient needs are met effectively.
- Establish a prominent clinical presence, providing leadership and clinical expertise to ensure high-quality patient care.
- Conduct comprehensive patient assessments utilizing advanced clinical skills.
- Independently prescribe medications in accordance with legal and professional guidelines.
- Develop, implement, and evaluate management plans for patients, employing advanced decision-making skills.
- Collaborate with multidisciplinary teams to provide ongoing patient support and follow-up care.
- Perform specialized diagnostic tests and treatments, demonstrating advanced assessment capabilities.
- Continuously assess and adapt care plans based on patient responses and evolving conditions.
- Act as a mentor and role model, promoting high standards of clinical practice.
- Engage in research and development initiatives to enhance service delivery and patient outcomes.
Communication and Collaboration
Utilize advanced communication skills to convey sensitive information to patients and their families, ensuring understanding and support throughout their care journey. Work closely with other healthcare professionals to foster a collaborative environment focused on patient-centered care.
Leadership and Development
Contribute to strategic planning and service improvement initiatives, ensuring alignment with local and national healthcare standards. Participate in training and development programs to enhance the skills of staff across various disciplines.
