An opportunity has arisen for a Skin Cancer care coordinator to work along side our Skin Cancer Clinical nurse specialists within Dermatology.

The purpose of this role is to support the Skin Cancer Team to implement the early diagnostic and treatment pathway for patients with skin cancer to achieve the 62-day cancer pathway. This role has been specially designed to support the optimal skin cancer timed pathway. The post holder will be involved from onset of the patients care pathway and will require to evolve to support the pathway as it continues to refine over time

In conjunction with the Clinical Nurse Specialists (CNS) provide coordination of high-quality patient care through on going telephonic and proactive identification of needs utilising basic clinical knowledge, approved tools and procedures

Main duties of the job

Collaborate with the CNS, Matron, pathway navigator and MDT team to ensure timely and effective treatment and care pathways in compliance with local, regional, and national guidelines. Identify areas for improvement, take appropriate action, and oversee pathway management related to 62-day treatment targets by monitoring hospital admissions and outpatient appointments.

To help with preparation of multidisciplinary team meetings by reviewing the patient's electronic health record to identify any previous or upcoming appointments within the dermatology-plastics clinic or with plastic surgeons, as per the MDT preparation trust policies.

To proactively action outcomes/plans from skin MDT meetings by booking appointments, delaying and cancelling appointments, working closely with pathway navigator. Ensuring patients are aware to attend appointments to avoid DNAs and inform CPBS to book appointment if appropriate. Upload any referrals or other correspondence from other hospitals/ primary care on electronic patient record

To collect data, produce findings, and develop presentations while documenting and disseminating outcomes from audit. Support the MDT in national and regional commitments, research activities, and preparing documents for publication and reports.

The post holder will improve patient experience by providing support and non-clinical information throughout the entire cancer pathway and reduce patient anxiety by providing reassurance and information on interventions/tests, surgery appointments and ensuring patients arrive for interventions fully prepared in the absence of pathway navigator.

To assist the CNS team in creating educational materials and resources for patients, professionals, and the public on skin cancer prevention and care. To prepare Macmillan patient information packs, order leaflets as needed. Manage timely reminders for resource review dates and oversee the maintenance of CNS MM and SCC databases

To prioritise and manage your own workload using initiative to meet deadlines, while utilising strong communication skills and basic clinical awareness. To provide cover when skin cancer pathway navigator is on leave. Ensure adherence to national and local standards for cancer management, referring complex issues to the CNS for assessment

Experience

Essential

Previous experience of working with the public and/ or patients in a healthcare environment Experience of interaction with all levels of staff Previous administrative and organisational experience and providing support to clinical team Experience of managing and prioritising own workload and working to deadlines Experience of dealing with non- routine and complex problems

Desirable

Experience in scheduling patient appointments and using hospital computer systems.
