Inter Provider Cancer Pathway Co-ordination Responsible for monitoring and tracking inter-provider transfers across organisational boundaries. Ensure all transfers are recorded accurately on the Cancer Tracking Database, PPM and PTL specialty. Proactively monitor and manage cancer waiting times by identifying risks and initiating corrective actions to ensure the standards set out in The NHS Cancer Plan and Cancer Reform Strategy are delivered where clinically appropriate. Liaise with internal Cancer MDT Co-ordinators, external staff in a variety of departments to ensure timely transfer of patients with completed mínimal datasets.

Negotiating with members/departments of external Trusts to ensure that patient appointments are booked to meet national and local waiting times targets as laid down in the NHS Cancer Plan. Inform potential patient breaches to the relevant General Managers including Cancer Services and Lung CBU. Contribute to developing action plans to prevent future breaches and delays Work alongside other Cancer MDT Co-ordinators to provide cover, support and share best practice Data Administration Ensure data is collected in real time as patients pass through their referral, diagnostic and treatment pathway to meet the national statutory waiting time targets and provide data that will support clinical staff in delivering quality patient care Collect data from a variety of sources e.g. EPR, patient casenotes, SMDT meetings, clinical and administrative staff, to ensure all documentation is adequately maintained, and accurately enter data into required computer databases including Waiting Times and clinical databases Assist in data collection, analysis and the production of reports when required for the Trusts Management Teams, Lead Clinicians, the Cancer Registry, Network Audit programmes and service agreements to meet the Trusts statutory requirements Ensure that all data is kept securely to maintain the confidentiality of staff and patients at all times Work to reporting deadlines and inform line managers in advance of potential problems Ensure that all data items are completed as necessary and in accordance with agreed data definitions so local processes and outcome data can be compared with national benchmarks In conjunction with SMDTs, review and support the implementation of new ways of working which may influence service changes/developments as the priorities for cancer modernisation change IT and Support Skills Use of Microsoft Office Setting up and use of video-conference equipment to support SMDTs Use the Cancer Tracking Database, PPM, PAS, (Pathology), Radiology, and other databases relating to the cancer pathway Interrogation of the cancer information system to: - Maintain data quality Provide analysis and reporting for clinical staff Provide performance related analysis and reporting To work within the provisions of the Data Protection Act of 1998 and the Caldecott report Escalation of Actions The post-holder will: Monitor the pathway timescales and escalate in a timely manner to achieve targets.

Ensure that all relevant information is provided, in a timely manner, to the staff responsible for initiating escalation and alerts. Timely review of responses and further escalation as required. Reporting Breaches of the Target The post-holder will: Identify breaches via the tracking systems, in accordance with the cancer wait guidance, and provide internal breach report and review of breach reason to the Cancer Business & Pathway Data Manager and General Manager. Ensure all breaches are identified and an initial report sent to members of the clinical team and General Managers as well as General Manager regularly.

Keep on record all breach forms and reasons for breaches for future audits. Communication The post-holder will: Gather information from various sources to ensure that the patient benefits from improved and speedier communication Communicate with key departments (diagnostics, clinic bookings etc.,) to ascertain if treatments conducted and to update records and databases Organise, coordinate and present patient information to the CBU and communicate aspects of care requiring completion to clinical staff at these meetings as required Communicate information on the patients progress on the care pathway to Specialty and General Managers and other staff as required. Liaise and communicate cancer guidance and definitions to key staff within the Trust.
