Job summary

The post holder will work as the lead Intravenous Access Specialist Nurse as an autonomous practitioner with responsibility for delivering a quality service to all staff and patients within the division and community setting in relation to intravenous access and education. Evidence based care will be inherent in their practice, ensuring that best practice is disseminated throughout the clinical areas. The role will be related to clinical expertise with an emphasis on initiating, performing and maintaining standards of clinical excellence.

The post holder will be a senior qualified nurse with an extensive record of nursing experience within the field of Intravenous Access and education. They will have a record of career long learning and education and they will require highly developed leadership, interpersonal relationship and communication skills. They will contribute to new knowledge by working collaboratively with the other practitioners within the organisation.

The main responsibilities are to have a significant role within the field of intravenous access and take a lead role in the educational requirements of the integrated services in respect to all aspect of classroom and clinical training. They will work alongside colleagues, maintaining good communication and collaboration between the various multidisciplinary teams.

Main duties of the job

Management of the intravenous access service.

Demonstrate expertise in the management of all central venous catheters including PICCs, Ports, Hickman catheters and apheresis catheters.

Develop and record a treatment plan consistent with the outcome of assessment and most probable diagnosis.

To be an expert practitioner in the skill of cannulation and PICC placement.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Management of the intravenous access service. To place peripherally inserted central catheters without supervision within their own clinic once deemed competent after assessment.. Provide patients with effective, evidence-based complication management of venous access devices and extravasation management. To take an active role in the writing/updating of policies and guidelines To have a highly visible clinical presence. Evaluate and plan appropriate intravenous access placement decisions when necessary. Demonstrate expertise in the management of all central venous catheters including PICCs, Ports, Hickman catheters and apheresis catheters. To demonstrate high level clinical expertise as an Advanced Practitioner and role model in the field of intravenous access. To use advanced clinical skills in the assessment and management of patients with complex intravenous access needs. Develop and record a treatment plan consistent with the outcome of assessment and most probable diagnosis. To be a referrer under the IR(me)R guidelines that will allow the radiological requests of X-rays and ultrasound scans in relation to placement and complication management of Intravenous access devices. Order first line diagnostic investigations biochemistry, haematological and radiological. Plan and manage own workload/caseload in a flexible manner, ensuring that service provision meets the needs of patients. To be an expert practitioner in the skill of cannulation and PICC placement. Demonstrates continuous evaluation of practice and make changes where appropriate. Ability to organise own workload and prioritise to meet deadlines and make clinical decisions independently.

To be involved in the planning and implementation of innovative practice.

Able to prioritise, organise, plan and implement workload plans ensuring maximum benefits of care are received and delivered at all times.

To actively seek to contribute to the growing knowledge base of oncology and intravenous access through publications and participation in conferences. To demonstrate an up-to-date knowledge of intravenous access practice and responsibility for ones own professional development. To maintain a portfolio to fulfil requirements of the four pillars of advanced practice. Maintain safe practice and development through regular peer review of evidence-based practice. Liaise and collaborate with multi-professional disciplines and outside agencies. To be aware of the policies, procedures and philosophies of the Trust, understanding health and safety requirements and ensure that all duties and responsibilities are carried out within these guidelines. Contribute to wider issues in the Trust, such as committee membership, clinical progression and policy development. Networking with senior nurses within other Health Boards in South Wales to produce guidelines and policies for the care and management of intravenous devices based on empirical evidence and best practice. Lead education and development programmes for healthcare staff within the department and beyond. Analyse complex data and initiate appropriate clinical management responses in collaboration with other key staff, clinical and managerial. Participate in clinical audit in respect to evaluating service effectiveness and future developments within the role. Monitor standards of nursing care and respond to quality and safety issues in clinical practice/care as required. Represent the Trust on regional and national forum, as required by the Head of Nursing VCC and the Executive Director of Nursing & service Improvement. To develop clinical protocols, standards and guidelines in own area. To actively contribute to the process of clinical risk management. Act as a role model to junior staff and encourage them to seek advice and solutions to problems. To provide clinical and professional support and advice to all staff. Develop and deliver specific and relevant teaching programs for all disciplines. Support, supervise and assess clinical competencies of nursing staff and other members of the multidisciplinary team. Maintain accurate records and provide necessary data for statistical analysis. Implement research findings appropriate to practice. Initiate new and support existing research projects. To raise quality issues and report related risks within the trust to address poor standards and incidents. To inform and influence Clinical Governance issues. Person Specification

Qualifications and Knowledge

Essential

Master's degree or evidence of working at that level to include history taking and clinical examination of patients Registered General Nurse -Currently registered with the Nursing & Midwifery Council. Teaching/ Mentorship qualification

Desirable

Independent Non-Medical Prescribing Qualification Advanced Life Support Course. MSc in Advanced Clinical Practice or willingness to undertake

Experience

Essential

Substantial experience of managing a variety Intravenous access issues. Ability to work across professional boundaries.

Experience in undertaking audit and management of change in practice related to audit results
