Job summary

We are seeking a motivated and inspirational Advanced Clinical Practitioner to join our UCR, and virtual ward team delivering care to prevent avoidable hospital admissions. The ACP will deliver practice working closely with an advancing practice team. Along with the Consultant Practitioner the foundation Consultant Practitioner you will take a lead with clinical responsibility for the patient caseload, and supporting trainee ACPs to develop the four pillars of practice. The ACP will take a lead role in clinical assessment of patients with highly complex acute and/or chronic disease presentations, making differential diagnosis and prescribing treatments as an independent prescriber if profession allows. We have 4 pathways;

Rapid response within 2 hours focuses on preventing avoidable hospital admissions for patients with acute ill health conditions by providing advanced clinical assessments that place the person at the core, This includes implementation of treatment and appropriate monitoring whilst in crisis.

UCR van consists of a frailty specialist alongside a Paramedic or ACP and responds to referrals from 999 for patients at risk of hospital admission through loss of independence and/or falls.

Frailty Virtual Wards supports patients who would otherwise be in hospital to receive the acute care and treatment in their own home.

Working with the ED Frailty Team in, supporting same day turn around, UCR/VW continue the patient journey from ED to home to reduce re-admission.

Main duties of the job

The post holder will be required to work with a high degree of autonomy and complex decision making underpinned by a masters level award that encompasses the four pillars of clinical practice, leadership and management, education and research with demonstration of core capabilities and area specific clinical competence.

They will manage clinical care in partnership with individuals, families and carers, which will include the analysis and synthesis of complex problems across a range of settings, enabling innovative solutions to enhance people's experience and improve outcomes. They will exercise autonomy and decision making in a context of complexity, uncertainty and varying levels of risk, holding accountability for decisions made.

They will work alongside, leading and clinically supervising the trainee ACPs to develop their four pillars of ACP expertise.

They will work collaboratively with the Consultant Practitioner or Trainee Consultant Practitioner to establish values based professional expert and evidence-based practice across pathways, services, organisations, and system working with individuals, families, carers, communities, and others.

Provide values-based leadership across the care pathway, services, and systems in complex and changing situations. Develop staff potential, add to, and transform the workforce, and help people to learn, develop and improve to promote excellence.
