Families often feel overwhelmed by the complexities of care, the coordination of multiple professionals and appointments, and the challenges of maintaining family life and employment. To support them, we developed our Hospice at Home Team, enabling families to manage their child's care in the comfort of their own home.

Rainbows Childrens Hospice cares for babies, children, and young people with life-limiting or life-threatening conditions, as well as their families, both at the Hospice and Hospital and, since the pandemic, in the community within their homes through our expanding outreach services.

Job responsibilities

About the Role:

As a Lead Band 7 Nurse, you will provide expert, comprehensive, and specialist nursing care to babies, children, and young people with life-limiting or life-threatening conditions, as well as respite support within their homes. You will collaborate with the Head of Palliative Care and Nursing and Clinical Transformation Lead to develop and implement innovative practices.

Key Responsibilities:

Maintain oversight of day-to-day prioritisation of care arrangements, including on-call arrangements and 7-day service delivery.

Manage referrals to the hospice at home service, working closely with the Rainbows MDT.

Assess and review clinical care and support, responding to changing physical and emotional needs.

Conduct assessments for referred children and families, developing and implementing specific care plans and ensuring timely reviews.

Lead and manage the Hospice at Home Team, upholding Rainbows Hospice's philosophy, values, and culture.

Requirements:

NMC Registered RNCB and educated to degree level.

3 years of experience working in the community with babies, children, and young people with palliative/end-of-life care needs.

Ability to work autonomously and as part of a team, commanding respect and remaining approachable.

Experience in managing and motivating a team with a desire to develop the Hospice at Home service.
