The successful candidate will be passionate about palliative care which puts each and every person and their individual circumstances at its heart. You will be interested in developing your palliative care skills further as you benefit from working alongside experienced end of life care professionals who have been serving the Central Lancashire community for more than 35 years.

Main duties of the job

Candidates must:

Registered on the NMC register with a minimum of 2 years post registration experience Be prepared to work night shifts when required, to meet the needs of the service. Be caring and compassionate Have well developed interpersonal skills Have or be willing to work towards a recognised qualification in palliative care Be enthusiastic and resourceful and keen to enhance their practice and skills Enjoy team working and have the ability to lead/coordinate a shift The successful candidate will also have the opportunity to maintain an NHS pension.

Job description

Job responsibilities

Assess, plan and implement, evaluate and oversee the delivery of patient centred care, using a holistic approach.

Take the role as Nurse Co-ordinator, as per rota leading the nursing team.

Make decisions particularly out of hours, concerning symptom management or deterioration in patient’s condition, contacting the on call medical team when needed.

Check and administer prescribed medication as per hospice policies, procedures and protocols.

Thoughtfully meet patients care needs.

Work within the Hospice and wider Multidisciplinary Team and take the lead with the discharge planning of patients.

Maintain accurate patient records in line with the Data Protection Act and NMC guidance, using electronic patient record system.

Be able to communicate effectively and sensitively with patients and families and other health care professionals.

Promptly report adverse events, accidents, near misses and errors via the DATIX system.

To have a working knowledge of hospice policies and procedures.

To maintain and update own clinical skills to deliver high quality care.

To attend education and training to maintain knowledge and skills.

Actively participate in annual personal development plan and be involved in appraising other members of the team.

Take the role of fire commander out of hours in the event of a fire.

Comply with registration requirements of the NMC

Person Specification

Qualifications

Essential

Registered Nurse on NMC register part 1 or 12 Willing to undertake Palliative Care Certificate.

Desirable

Palliative Care Certificate Diploma in health and social care Mentorship/Teaching and assessing in clinical practice

Experience

Essential

Two years post registration Experience of taking charge of a shift and managing people (Nurse coordinator role)

Desirable

Have provided palliative care to patients in other settings

Other

Essential

Resilience to work effectively in a palliative care setting

Key Skills & Abilities

Essential

Demonstrate proficient skills in written and verbal communication in the English language Have the emotional resilience to work effectively in a palliative care setting and cope with care of patient who are dying and their care after death Basic understanding of drugs commonly used in palliative care/end of life care Evidence of professional development, self-motivation and initiative Professional, sensitive style of communication and possess good IT skills Able to prioritise and manage work load effectively whilst working flexibly and as part of a team Maintain confidentiality at all times and ensure colleagues uphold the same standard Promote equality and diversity

Desirable

Good knowledge of pain and symptom management Good understanding of health and safety issues in a clinical environment Have a good understanding of the Mental Capacity Act 2005
