Working Style: 

The Older Adult Mental Health Team North is based in Colchester, covering both Colchester and Tendring, and the team has an exciting opportunity for a community support worker to join the team.

As a Community Support Worker, your role will include working with and managing a caseload of adults aged 65 years and over diagnosed with a functional or organic mental illness such as dementia; and their families both in the community by undertaking and writing assessments and reviews in line with our Statutory duties under the Care Act 2014 principles to identify eligible needs.

The role includes generating person-centred and outcome focused support plans; and developing discharge pathways from inpatient wards in accordance with the Mental Health Act 1983 and Mental Capacity Act 2005. 

An essential attribute for the role is to be ‘curious’ as thinking ‘outside the box’ is a must to achieve the best outcomes and to fully engage and utilise all that our community has to offer. Other roles include supporting with the teams duty function, supporting as appropriate with safeguarding intervention and multi-agency working.

Partnership working with both internal and external partners is essential to achieving the best outcomes for Adults and Carers. As a Community Support Worker, you will need to work proactively to develop sustainable relationships with internal and external colleagues.

The work is varied and interesting. An induction and appropriate training will be provided. You will also be joining a supportive team that encourages development and growth. It is also really important that you demonstrate a commitment to learning and development to support yourself and the team to deliver the best service possible.

About You

You’ll bring a strong ability to build effective relationships with adults and their families in times of challenge and crisis. You will have the drive and passion to want to make a difference, and a willingness to learn. Ideally you will have some experience of activity-based work with adults and some knowledge of legislative and policy guidance applicable within social care settings.

Please note that the job role requires you to be mobile throughout an operational area. Therefore, the post holder will need to have a driving licence and access to a vehicle .
