Overview

Change Grow Live are a charity dedicated to the belief that we can make a difference to our Service Users lives. You will work as part of the team, working within the wider community engagement team, which provides a range of peripatetic services within the area. This role will primarily work within local primary care spaces such as GP surgeries, hospitals, pharmacies, and other community settings.

You will provide support to people that have been identified as requiring help with addressing their alcohol and/or substance misuse. You will lead on a range of interventions and provide a package of holistic support to help build resilience and improve people’s ability to achieve their goals and build on their recovery. You will also lead on establishing professional relationships with the local Primary care network.

Where: Newham

Hours: 37.5

Salary: £31,529,41 to £33,708,57 Pro rata dependant on experience

Allowances: Inner London Weighting (£4,072.06)

Responsibilities

About The Role:

Provide psycho-social interventions within the primary care setting to those who are not currently accessing the core hubs. To work collaboratively with the professionals in Primary Care settings to
provide support to those with alcohol and/or substances misuse issues Working with the professionals in Primary Care to ensure the people we support have regular health checks including BBVs and other relevant screenings where required. Carry out comprehensive assessments and develop recovery plans that clearly identify how they will enable progression in addressing substance use and re/integrating into the community. Engage in regular consultation with service users to maintain the quality of the service, supporting staff by encouraging service user engagement.

About You:

Ability to build strong professional relationships within Primary care. Ability to work autonomously and collaboratively with Primary Care professionals and build links between services. Knowledge of a range of interventions including preventative measures, harm reduction strategies, motivational interviewing, and other psycho-social interventions. Excellent knowledge and understanding of drug and alcohol issues and experience of working within a related field. Knowledge and experience of multi-agency working and establishing partnerships with other professions to achieve good outcomes for the service user group, . mental health services, criminal justice agencies, community groups, etc.
