I am getting in touch as Baltimore are working with a London based local authority in their search for a highly skilled health professional to join the authority and support as their Director of Primary Care Transformation on an interim basis.
 
This position will be working directly with the Network management leads, Clinical Directors and ICB / ICP senior leaders and partners, supporting the ICB as they transition into more streamline pathways following a restructure.
 
Your duties in this position:
Deliver on priorities highlighted by Networks, PCNs, Council Teams and ICB Partners to delivery smarter pathways and ways of working.
Support the integration of General Practice with eh wider Primary Care and Community Care network. This will support at scale delivery of services that will benefit the wider population.
Work with population health in the transformation and delivery of care improvement priorities to tackle existing and future health inequalities.
Deliver on Health & Care Act aspirations, supporting effective integration with partners to ensure governance and assurance.Experience needed for this position:
Strategic understanding and extensive experience of working in the NHS with knowledge and understanding of organisations and the primary care sector.
Experience of developing provision at scale of primary scale, working with local practice and strategic leaders.
Excellent communication skills with the ability to work with a variety of partners and sectors.
Experience of working at a senior level within local government or the NHS.
Be highly resilient.
