Primary Care Transformation Lead

We are seeking a highly motivated and experienced professional to join our team as a Primary Care Transformation Lead. This is an exciting opportunity to work closely with our general practices, Primary Care Networks, GP federation, and wider health and care partnership across Greenwich.
Main DutiesThe successful candidate will play a key role in developing and implementing primary care transformation initiatives, working closely with our colleagues in the Healthier Greenwich Partnership and across the South East London health and care footprint. Key responsibilities will include:
- Developing and maintaining collaborative relationships with general practices, Primary Care Networks, and other stakeholders to drive primary care transformation.
- Leading or supporting primary care improvement initiatives, service or pathway redesign, and procurements to deliver transformation in Greenwich and South East London.
- Participating in patient, public, and carer engagement as required, through a range of different engagement and communications methods.
- Staying up-to-date with health and care related policy development and initiatives, and contributing to South East London and Greenwich strategy and planning as required.
- Deputising for colleagues and leaders in the Primary Care team or more widely as required, and representing Greenwich borough or South East London at external meetings and events.
The NHS South East London Integrated Care Board is an NHS statutory organisation, bringing together partners involved in planning and providing NHS services. We are responsible for allocating NHS resource to deliver the Integrated Care Partnerships strategy.
Person SpecificationThe ideal candidate will have:
- Strong communication and relationship-building skills, with the ability to develop mutually productive relationships with peers and senior colleagues alike.
- Financial competence, with the ability to understand budgets and costs and treat public money as their own.
- Strong project management skills, with the ability to manage tasks and end-to-end projects.
- Able to analyse data and multiple sources of qualitative and quantitative information relating to health and care services and patients and population health.
- Familiar with common research methodologies, such as surveys, focus groups, literature reviews, and online research tools.
- Self-motivated, able to work autonomously, and able to ask for help when needed.
- Good time management, with the ability to manage a portfolio of projects and duties within the team.
- Project a positive and professional image of Greenwich ICB, Greenwich Health, and the wider primary care and HGP landscape in Greenwich.
- Uphold confidentiality, good information governance, and exercise discretion on sensitive matters.
- Adhere to all mandatory training requirements of the organisation.
