Sherborne, United Kingdom Dorset HealthCare University NHS Foundation Trust Full time

Job summary

Come and join our Dorset Stroke and Neuro Service serving the West and North of Dorset. We have recently been awarded significant investment in Dorset to develop Stroke and Neuro Services across Dorset to meet the National Stroke Service model published by NHS England last year (particularly with regards to Integrated Community Stroke Services- ICSS). This is a really exciting time as we significantly expand our existing services and embed innovative ways of working to improve clinical outcomes for patients following stroke or neurological illness or injury. Importantly this once in a generation investment increases our team threefold and significantly extends the time patients can access the service moving to a needs led, rather than time limited service. Our integrated community stroke and neuro team is AHP lead and will provide various pathways of care including supported self management, advice and signposting, intensive rehabilitation (ESD), complex management and reviews.

Do you embrace change? Do you want to be part of building something exciting? Do you thrive on new knowledge, evidence based practice and research? Come and join us.
