Job summary hours per week permanentThere is an exciting opportunity to join, theMental Health Assessment andIntervention Service (MAIS).
The service was set to provide high quality carefor people experiencing mental health problems in Plymouth, and has proved asuccess across the city.
The service will offer evidence-based treatmentpathways for people in Primary Care Services.
The role will develop moreeffective links with primary care and third sector agencies to support thosepeople with less complex mental health problems.
We are seeking to recruit an experienced andflexible Mental Health Nurse with excellent networking skills to join the team.This post will offer a challenging and exciting opportunity to gain experienceof working across Livewell's Core mental Health Service, working across avariety of settings such as GP surgeries, and Community hubs developingrelationships with Primary Care, partner agencies and secondary mental healthservices.
You will have the opportunity to design and deliver evidence basedrecovery focussed mental health interventions and groups.
The CMHS pathway provides a variety of opportunities for clinical support and development, including a tailored induction programme, regular multi -disciplinary team meetings, monthly individual supervision, and regular multi-disciplinary caseload supervision.
As a member of the Core mental health teams, you will also contribute to service developments at the quarterly meetings.
Main duties of the job The post holder will be responsible for maintaining high standards of clinical services to patients.The post holder will behave in a manner which does not bring the service and Livewell Southwest into disrepute.The post holder will implement decisions agreed by the Locality, including all policies of Livewell Southwest.The post holder must have completed training hold a currently valid registration with the relevant body for their profession to a degree level.
Have minimum of 2-year post qualification experience.Salary will be Band 6.The post holder will need to travel and attend to service users based across Plymouth and other geographical areas.
Full and valid driving licence and access to a car for work purposes is required.

Job description Job responsibilities ServiceDelivery:The Mental Health Assessment andIntervention Service (MAIS) is an integral part of the Community Mental Health Servicespathway.
MAIS is a practitioner led service with access to consultation andsupport from psychologists and psychiatrists.
The team have close links withprimary and secondary care and third sector agencies.
As part of the CMHS pathway the team contributeto the bio-psycho-social assessment process for newreferrals to determine eligibility for secondary mental health care services,providing consultation and advice to referrers and other agencies regardingalternative appropriate services when the patient does not need the support ofthe CMHS pathway.The team provide short-term evidence-basedrecovery focused interventions on an individualand/or group basis for people experiencing mental health disorders whosneeds cannot be met within universal services and who do not require a full multi-disciplinaryapproach to their management.
The teamwork with a defined group ofpatients under standard care, this will include the assessment of need,planning, implementation and evaluation of care, the assessment and managementof risk, consideration, and management of safeguarding issues.
The post holderwill be responsible for any onward referrals that are required and work closelywith all other clinical professionals , Consultant Psychiatrist, ClinicalPsychologist, Social Worker and third sector agencies etc.
The team will support the Primary Careand CMHS Team Leaders to establish robust working practices within Primary Caresettings, third sector agencies and other Livewell Services.ClinicalThe post holder will, in conjunction with otherstaff from MAIS, provide initial assessments and support calls within the GPsurgeries to determine eligibility for secondary mental health care servicesand or intervention.
They will also provide consultation and advice toreferrers and other agencies regarding alternative appropriate services whenthe patient does not need the support of the CMHS pathway.The post holder will provide evidence-basedrecovery focussed interventions for patients on an individual or group basis inconjunction with our partner agencies.
Deliver psychoeducational packages andtraining related to the management of common mental health disorders to supportthird sector agencies to assist them to support people with common mentalhealth problemsThe post holder will advise, in conjunction withDrs and the Nonmedical prescriber, on matters relating to medication within theCMHS.The post holder will provide consultation toprimary care and third sector agencies to ensure that those people who requiremental health support are identified and are referred onto the appropriateservices in a timely manner.The post holder will work closely within PlymouthPrimary Care Networks (PCNs), GP surgeries to provide assessment, support andguidance for those people who present frequently or present with complexproblems but who do not meet the criteria for secondary mental health care.The post holder will be involved in the triaging ofmental health work in GP surgeries and educating primary care staff on the roleof MAIS.Service DevelopmentThe post holder willwork with MAIS to develop interventions which support the CMHS to become NICEguideline compliant.The post holder will be required to attendany relevant working groups as directed by the manager in relation to theeducational aspect of their role.The post holder will work under the direction of the manager/ practicelead to agree how the education packages will be delivered.The post holder will support the development and delivery of trainingfor patients, carers, and partner agencies in agreement with the manager.The post-holder will takepart in the clinical audit of the team.
This will include annual Recordsaudits.Staff Management:Thepost holder will be responsible for supervising junior staff within the CMHS,including line management of junior staff.The post holder will support the induction ofnew staff across the CMHS serviceThe post holder will act as a mentor forstudents and new staff within the team.Liaison and Administration:The postholder should maintain close links with all other Livewell Services asrequired.The postholder will develop and maintain links with primary care and third sectoragenciesThe postholder will keep up to date electronic/paper records of all patient and staffactivity, ensuring these are kept in a safe place, in accordance with their ownprofessional body guidelines, and Livewell Southwest policies including recordkeeping, confidentiality, Data Protection Act and the Mental Health Act andprofessional conductThe postholder will be aware of their professional and Livewell Southwest guidelines onMental Health Act and Data Protection Act.Professional Development and Education:The postholder will be required to keep up to date this will include Mandatory trainingand developments in mental health care.The postholder will participate in the provision of relevant community placements andtraining programmes for Mental Health professionals.The postholder will be required to ensure that they receive appropriate regular linemanagement and clinical supervision in line with current policies of LivewellSouthwest.The postholder will participate in an annual appraisal; this Job Description will bereviewed at this time.PHYSICAL DEMANDS OF THE JOB This role will involve regular time spentusing computerised systems this will include the working of GP systems.
It will involve travel across the citythroughout the day.
The post may involve carrying equipmentfrom one area to another to support the delivery of training.
Lone working will be involved in a varietyof settings including patients homes, and community settings.

Person Specification Qualifications Essential RMN/Dip HE/BSc Mental Health Nursing- NMC Registration.
Occupational Therapy Degree HCPC Registration.
Social Work Degree or PG Dip or MA Social Work England Registration Experience Essential Relevant post registration experience.
