Job summary The main aim of the role is to deliver specialized nursing care to patients with IBD, including clinical consultation for inpatients and outpatients.
The candidate will need to develop advanced clinical skills of history taking, assessment, diagnosis, and prescribing.
Main duties of the job Advanced Clinical Practitioner in Inflammatory Bowel Diseases (IBD).
Provide comprehensive clinical skills, information, advice and support to patients with IBD.
Work as an independent practitioner alongside other members of the multidisciplinary team.
Administer, assess and prescribe specialist medication.
Participate in clinical audits in specialist area.
Participate in the education and training of the multidisciplinary team within the trust, regionally, nationally and internationally.
Provide expert clinical advice and support to all members of the multidisciplinary team.
Be flexible within the service to ensure appropriate cover.

Job description Job responsibilities Clinical Practice Health Promotion, Protection and Prevention Assess and address patient health education needs for all aspects of care from positively influencing mindset at new diagnosis to support and facilitation of transition through critical periods.
Use highly developed interpersonal and communication skills in programme development especially in key areas smoking cessation in Crohns disease.
Appropriately use and promote the use of key digital resources such as the True-colours patient self-assessment tool; utilising and integrating this tool with own clinical practice.
Assessment of Patient Illness and Clinical Decisions Assessment of inpatients and outpatients via face-to-face, telephone and email and participate in relevant clinics as first point of contact.
Assess and support IBD inpatients Act as patient advocate at all times.
Apply advanced clinical history taking to receive, analyse and interpret complex medical and surgical information.
Diagnostic skills will apply.
Apply relevant advanced physical examination skills such as abdominal and peri-anal examination.
Appropriately arrange or agree investigation and intervention for prevention, assessment, surveillance and treatment.
Formulate a plan of care for patients involving and empower them to make informed decisions.
Liaise with multidisciplinary team involved in direct patient care such as the gastroenterology consultants Record and document treatment appropriately, including to practitioners in other specialties and primary care.
Utilise and champion the integration of appropriate patient-reporting tools such as True-colours within own practice as well as the wider team Interventions and Treatment Administer, train and monitor specialist I/V and SC medication.
Ensure safe use of specialist medication including oral treatments through timely clinical assessment.
Utilise pharmacology and pharmacokinetic knowledge through competencies to accredited non-medical prescribing.
Implement (prescribe), evaluate and modify treatment for optimal outcome considering referral to other specialists as appropriate.
Develop links with other areas within the trust and community including dieticians, GPs and district nurses, to facilitate continuity of care for patients.
Record and document appropriately including dissemination to practitioners in other specialties and primary care.
Leadership and Management o Act as a role model to all staff, providing clinical support and advice and demonstrating high standards of nursing expertise and patient care.
o Is responsive to, and considerate of, the gastroenterology unit as a whole when addressing service needs and leading improvement initiatives.
o Provide cover in the absence of ANP colleagues o Promote the role of the IBD nurse within the directorate, trust and national arena.
o Remain aware of organisational goals and trust objectives when managing the service Education and Training Development of self Understands own accountability and responsibility working within NMC guidance and maintains professional registration.
Achieves masters level accredited education in relevant aspects of Advanced Nursing Practice for IBD nursing.
Undertake personal development to meet identified needs continually updating nursing practice in accordance with current research and evidence in specialist practice.
Actively engages in peer network for collaboration and support, information and learning.
Pursues opportunities for this at a local, regional and national level.
Attend mandatory training and e-learning activities.
Patient and Carer Education and Training Provides appropriate education to patients, families and carers in person as well as via the Advice Line Develop written education and consider other possible communication methods such as educational programmes and support groups.
Ensure patients understanding: empowering and encouraging involvement towards self-management and self-monitoring.
o Development of Others Provide clinical advice and support for nurse specialists within the department and the trust.
Contribute to colleague development through disseminating own knowledge and facilitating nursing student experience.
Staff education enables unified response such as the need for smoking cessation for Crohns patients.
To act in an advisory and specialist clinical capacity in partnership with medical, nursing and paramedical staff within the trust, community and at a national level.
This includes active involvement in appropriate professional and charitable organisations.
To be skilled at making presentations using multimedia tools at local and regional meetings to all members of the multi professional team to share best practice.
Teach on a range of study days to members of the multidisciplinary team on IBD, including affiliated education programmes.
Publish and present research findings locally, nationally and internationally via relevant journals and conferences.
Assist in the collection of information and maintenance of the departmental database for in/out patients.
Demonstrate competent IT skills Contribute to the appraisal needs of nursing and multidisciplinary colleagues as required.
Improving quality and the development of practice Actively promote diversity and equality Contributes to the lead of the service and developing own projects according to service needs.
Identify and appropriately respond to areas of risk through governance structures and participate in a team approach.
Involve patients in development through discussion, survey, groups and charities such as UK Crohns and colitis local group.
To liaise with pharmaceutical companies and charitable organisations appropriately.
Work alongside the whole multidisciplinary team to agree and implement guidelines and protocols as they develop.
Develop protocols for own projects.
