Ibd Clinical Coordinator

Job overview An exciting opportunity has arisen to join the small nursing team delivering care to patients with Inflammatory Bowel Disease (IBD: Crohn's disease and ulcerative colitis).This is an interesting and varied post involving face to face as well as remote care, primarily in the coordination of IBD nurse interventions.
Your input will be in specialist immunological treatments, managing the home testing programme, surgical pathways, patient information materials and other projects that give additional breadth to the role.
You will have support from the IBD Nursing team and, where needed training can be discussed around skills such as venepuncture, cannulation, and following agreed plans of care.
Personal attributes you will need are excellent organisational and communication skills.
You must also be proactive and confident in discussions with patients and the multidisciplinary team to ensure safe and compassionate patient care.
From a practical perspective this job would suit someone who is able to work within school hours each day and spread the hours over the Monday – Friday week.
Main duties of the job .
Work across departmental boundaries to coordinate with day case, pharmacy, surgery and administration to ensure patients follow appropriate pathways Ensure clinical areas are resourced and work alongside the IBD nursing and day case teams to deliver advanced therapies; cannulating, monitoring and taking blood testsCoordinate care through key transition points such as surgery, paediatric to adult care, home and hospital and between different treatmentsManage home testing services; processing referrals, results for faecal home calprotectin and ensuring completeness of communication Working for our organisation Oxford University Hospitals NHS Foundation Trust is one of the largest NHS teaching trusts in the country.
