About the Opportunity

We are seeking a highly motivated Community Impact Coordinator to join our team. As a key member of our team, you will work closely with internal and external stakeholders to deliver high-quality support services to residents with additional requirements.

This is a challenging yet rewarding role that requires someone with excellent communication and interpersonal skills, as well as experience in caseload management and monitoring. The successful candidate will be responsible for building relationships of trust with residents, advising and empowering them to achieve their goals.

Activities will include carrying out comprehensive assessments of residents' circumstances and supporting them to complete action plans that address all immediate, medium- and long-term needs. The ideal candidate will have a strong understanding of the challenges faced by residents with additional requirements and be able to provide solutions-focused support.

Key Responsibilities
  • Deliver high-quality support services to residents with additional requirements.
  • Build relationships of trust with residents and empower them to achieve their goals.
  • Carry out comprehensive assessments of residents' circumstances and develop action plans to address their needs.
  • Work closely with internal and external stakeholders to ensure effective delivery of support services.

Salary: £40,915 per annum

Location: South and South London
