Job Description

The Integrated Neighbourhood Team Coordinator plays a vital role in facilitating community-based activities within health and social care in their respective neighbourhoods. They will work closely alongside existing services, including health, social care, and Voluntary, Community and Social Enterprise (VCSE) organisations.

Key Responsibilities
  • Becoming a local expert, understanding the strengths and challenges of the Neighbourhood
  • Project managing priority programmes for the Integrated Neighbourhood Team, bringing the right partners together to deliver, tracking process, impact, and risk management

Person Specification
  • Educated to Master's degree level or equivalent combination of postgraduate qualification courses and experience
  • Highly developed health and care knowledge acquired through relevant degree plus additional training or equivalent experience
  • Can evidence recent and ongoing personal development activity
Experience
  • Demonstrated experience of coordinating multiple projects
  • Current broad knowledge of NHS and Local Authority policy and context
  • Experience of scheduling/coordinating activities and/or resources such as care planning
  • Evidence of managing projects with the aim of delivering service
  • Experience of managing a project budget
  • Experience of managing risks and reporting
  • Experience of working in a multi-agency environment, developing and maintaining relationships across services and supporting them to work together to achieve coordinated responses to citizens' needs
  • Experience of setting up and implementing processes and procedures
  • Experience of working in a diverse community
  • Experience of managing change within a multi-agency working environment, with front-line, middle management and/or senior management level
  • Experience of delivering training
Skills & Knowledge
  • Understanding of health and social care issues for vulnerable people and how these relate to health inequalities
  • Understanding of the principles of Multidisciplinary Team (MDT) working
  • ICT skills, including standard keyboard skills and ability to use Microsoft Suite (Word, Excel, PowerPoint and MS Teams)
  • Able to build and sustain positive relationships with a wide range of stakeholders, including citizens, carers, Voluntary and Community Sector organisations and health and care professionals
  • Ability to engage, persuade, empower, motivate and influence individuals and partner agencies
