Care Planning Specialist

About the Role: We are seeking a highly skilled Care Planning Specialist to join our team in Tameside. As a key member of our Children Care Planning & Adoption Team, you will be responsible for managing complex care plans and supporting adoptive placements.

Key Responsibilities:

  1. Experience writing Child Permanence Reports, Court Reports, and all documents relating to the linking, matching, and placement of children for adoption.
  2. Family finding, linking and matching children, attendance at Panel as well as supporting adoptive placement.
  3. Direct work with children, preparing children for transitions and moving to their adoptive placements.
  4. Manage complex care planning.
