Job Summary

We are seeking a highly motivated and compassionate Support Worker to join our team in Gravesend. As a Support Worker, you will be responsible for providing high-quality care and support to individuals with complex needs, including those with learning disabilities and mental health conditions.

Key Responsibilities

  • Provide person-centered care and support to individuals with complex needs
  • Assist with daily living tasks, such as personal care and household chores
  • Support individuals to engage in activities and hobbies that promote their physical and emotional well-being
  • Work collaboratively with other healthcare professionals to develop and implement care plans
  • Maintain accurate records and reports, as required

Requirements

  • Experience working with individuals with complex needs, including those with learning disabilities and mental health conditions
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Basic knowledge of health and safety procedures
  • Enhanced DBS check
