Job Title: Senior Clinical Pharmacist - Primary Care Network

Job Summary:

The Senior Clinical Pharmacist - Primary Care Network will be employed by Greenwich Health Ltd, a leading healthcare organization in the UK. The successful candidate will work with a Primary Care Network (PCN) to deliver high-quality patient care and improve health outcomes.

Key Responsibilities:

  • Proactively manage patients with co-morbidities and polypharmacy
  • Provide clinical leadership on medicines optimisation and quality improvement
  • Manage the repeat prescribing process and ensure patients receive the medicines they need
  • Work with hospital colleagues to proactively manage patients at high risk of medicine-related problems
  • Provide patient-facing clinics for those with questions, queries, and concerns about their medicines
  • Manage medicines at discharge from hospital and ensure continuity of care
  • Develop and implement practice searches to identify cohorts of patients most likely to be at risk of unplanned admissions and readmissions from medicines
  • Ensure that patients are referred to the appropriate healthcare professional for the appropriate level of care within an appropriate period of time

Requirements:

  • Masters degree in Pharmacy
  • Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience
  • Minimum 5 years post-qualification experience
  • In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare
  • An appreciation of the nature of GPs and general practices
  • Registration with the General Pharmaceutical Council

Working Relationships:

  • Patients and carers where appropriate
  • GPs, nurses, and other practice staff, including practice pharmacists
  • GP prescribing leads
  • CCG pharmacists and medicines management team
  • Secondary Care and interface Pharmacists
  • Community Nurses and other allied health professionals
  • Community Pharmacists and support staff

Health & Safety:

  • Assist in promoting and maintaining their own and others' health, safety, and security as defined in the practice Health & Safety Policy

Equality and Diversity:

  • Support the equality, diversity, and rights of patients, carers, and colleagues

Personal/Professional Development:

  • Participate in any training programme implemented by the PCN as part of this employment

Quality and Governance:
