About the Role

We are looking for a skilled Holistic Care Coordinator to join our team at Centene Corporation. As a key member of our care management team, you will play a critical role in ensuring that our members receive high-quality, person-centered care that meets their unique needs and circumstances.

Key Responsibilities

  • Evaluating the needs of our members and identifying the resources available to support them.
  • Developing and implementing personalized care plans that address the specific needs and goals of each member.
  • Collaborating with healthcare providers, family members, and caregivers to ensure that our members receive the support they need to maintain their health and independence.
  • Maintaining accurate records of member information, care plans, and progress towards goals.

Requirements

  • A Bachelor's degree in a relevant field such as social work, gerontology, pediatrics, or human services.
  • 2-4 years of experience in care management, social work, or a related field.
  • Prior experience with LTSS and/or HCBS coordination, care delivery monitoring, and care management is highly desirable.

  • A competitive salary range of $25.97 - $46.68 per hour.
