Adult Care Coordinator

We are seeking a skilled and dedicated Adult Care Coordinator to join our team at County Durham & Darlington NHS Foundation Trust. As a key member of our healthcare team, you will be responsible for coordinating adult care services and ensuring high-quality patient outcomes.
About the Role:
This is an exciting opportunity to work in a dynamic and supportive environment, where you will have the chance to develop your skills and experience as an Adult Care Coordinator. You will be responsible for assessing adult patients, planning and delivering nursing care that reflects current patient care needs, and participating in teaching and training junior staff members.
Key Responsibilities:
- Prominently assess adult patients' conditions and develop individualized care plans.
- Co-ordinate and deliver exceptional nursing care that meets the highest standards of quality and safety.
- Participate in training and development opportunities to enhance your skills and knowledge.
- Ensure compliance with departmental policies and procedures.
Requirements:
To be successful in this role, you must hold a valid NMC registration and have a strong understanding of adult care principles and practices. Additionally, you should possess excellent communication and interpersonal skills, with the ability to work effectively within a team environment.
Salary and Benefits:
The salary for this position is estimated to be around £35,000 - £42,000 per annum, depending on experience. In addition to a competitive salary, we offer a range of benefits, including access to ongoing training and development opportunities, a comprehensive induction programme, and a supportive work environment.
