Care Coordinator for Adults with Learning Disabilities

We are seeking a motivated and compassionate Nursing Associate to join our team in the Intensive Support Service. As a Care Coordinator, you will play a crucial role in supporting adults with learning disabilities who have complex physical and emotional health needs.
The successful candidate will be responsible for delivering high-quality care and enhancing community services, working with individuals who require specialist interventions using trauma-informed care and a recovery-based approach.
You will work as part of an multi-disciplinary team, addressing and supporting the health, psychological, emotional, and behavioral needs of adults with learning disabilities.

Key Responsibilities- Care coordination and planning for adults with learning disabilities
- Delivery of high-quality care and support in line with individualized plans
- Collaboration with other healthcare professionals to ensure seamless care pathways
- Maintenance of accurate records and reports
- Registered Nursing Associate on the NMC register
- GCSE Grade A-C in Maths & English or skills level 2 Maths & English or equivalent
- Qualification in assessment, supervision, and teaching in clinical practice
We offer a competitive salary of £26,530 - £29,114 per annum (pro rata) and a range of benefits including a trust car lease scheme.
This role is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975, and as such, it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
