Job Summary

We are seeking a skilled Patient Pathway Navigator to join our team. As a key member of our healthcare team, you will play a vital role in ensuring the smooth flow of patient care and treatment.

Key Responsibilities
  • Monitor and Coordinate Patient Care: Utilize the Patient Tracking List to monitor patient care and treatment progress, adhering to Trust and local policies.
  • Collaborate with Clinicians and Managers: Work closely with clinicians and managers to identify and resolve issues that may impact patient care and treatment.
  • Ensure Efficient Patient Transfer: Coordinate the timely and efficient transfer of patients and information between trusts and other organizations involved in the patient pathway.
  • Develop Working Relationships: Foster strong working relationships with a wide range of clinical and managerial staff to ensure effective communication and collaboration.
  • Analyze and Report Issues: Work with the team to analyze target breaches and report issues to management, ensuring prompt resolution and improvement of patient care and treatment.
Requirements

To be successful in this role, you will need to possess excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment. You will also need to be able to analyze complex information and make informed decisions to drive improvement in patient care and treatment.
