About the Role

We are seeking an experienced Senior Care Assistant to join our Mental Health Services for Older People team in Hertfordshire.

As a key member of our social care team, you will be responsible for supporting patients with their journey back to full health. You will work under the supervision of a healthcare professional and be accountable for assessing patient needs, planning, implementing, and evaluating care.

You will also be expected to develop programmes of care without direct supervision, providing advice and guidance to clients and their carers, and participating in quality care projects.

The ideal candidate will have excellent communication skills and be able to challenge practices that compromise high-quality care for older people.

Key Responsibilities:

  • Assess patient needs and develop individualized care plans
  • Provide support and guidance to patients and their carers
  • Participate in quality care projects and contribute to service improvement
  • Develop and implement programmes of care without direct supervision

Requirements:

  • Experience working in mental health services or a related field
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
